The state defines "Homeowner" as, " Person(s) who owns a parcel on which he/she resides or intends to be, a one or two family dwelling, attached or detached structures accessory to such use and/or farm structures. A person who constructs more than one home in a two-year period shall not be considered a home owner." The building department for the City of Northampton wants person(s) who seek to use the home owner exemption, to act as their own construction supervisor, to be aware that by doing so you become responsible for compliance with state building codes and regulations. The inspection process requires that the building department be called to inspect work at various stages, which include foundation/footings (before backfill). sonotube holes (before pour), a rough building inspection (before work is concealed), insulation inspection (if required) and a final building inspection. The building department requires these inspections before the work is concealed, failure to secure these inspections can result in failure to obtain a certificate of occupancy until the work can be inspected. If the homeowner hires other trades to perform work (electrical, plumbing&gas) the homeowner will be responsible to make sure that the trades hired secure their proper permits in conjunction to the building permit issued, and that they get their required inspections.Failure of the individual trades to secure the permits and inspections as required can DELAY the project until such time as the proper permits and inspections are made I, understand the above. me owner/resident's signature requesting exemption) I will call to schedule all required building inspections necessary for the building permit issued to me. Hampton, MA Dear Mr. Willard: At your request, Western Mass Environmental (WME) has completed this letter to document testing from an area of a former approximate 1,000 gallon gasoline underground storage tank (UST) at the above referenced property. WME personnel were present at the property on December 16, 2008, to oversee test pitting activities from the approximate location of the former UST. A test pit was completed to approximately 10 feet an soil samples were collected. Samples were screened with a photoionization detector (PID) calibrated with isobutylene span gas standard "as benzene" and utilizing the Massachusetts Department of Environmental Protection (MADEP) "jar headspace" screening procedures. The samples were also inspected for visual and olfactory indications of petroleum impacts. Neither visual nor olfactory observation or PID readings were detected that would indicate any petroleum impacts. In an effort to be conservative soil sample was collected from the base of the excavation (ID: "TP-1"). The soil sample was submitted under chain of custody to Spectrum Analytical, Inc. in Agawam, MA for analyses of volatile petroleum hydrocarbons (VPH) and target gasoline parameters. The results indicated no concentrations in excess of laboratory method detection limits and thus were well below applicable reportable concentrations as outlined in the Massachusetts Contingency Plan (MCP). Therefore, in the opinion of WME no further action is required with regard to this UST. Copies of the analytical data are attached to this letter. Should you have any questions with regard to this letter please do not hesitate to contact the undersigned. The spike recovery was outside acceptance limits for the MS and'or MSD. The batch was accepted based on acceptable LCS recovery. VC10 The VOC preserved soil sample is not within the 1:1 weight to volume ratio as recommended by SW846 methods 5030 and 5035 but may be within the 1:1 volume to volume ratio. This variance may affect the final reporting limit BRL Below Reporting Limit-Analyte NOT DETECTED at or above the reporting limit dry Sample results reported on a dry weight basis N R Not Reported RPD Relative Percent Difference J Detected but below the Reporting Limit;therefore, result is an estimated concentration(CLP J-Flag). A plus sign(+)in the Method Reference column indicates the method is not accredited by NELAC Laboratory Control Sample(LCS): A known matrix spiked with compounds)representative of the target analytes, which is used to document laboratory performance Matrix Duplicate: An infra-laboratory split sample which is used to document the precision of a method in a given sample matrix Matrix Spike: An aliquot of a sample spiked with a known concentration of target analyt4s). The spiking occurs prior to sample preparation and analysis. A matrix spike is used to document the bias of a method in a given sample matrix Method Blank: An analyte-free matrix to which all reagents are added in the same volumes or proportions as used in sample processing. The method blank should be carried through the complete sample preparation and analytical procedure The method blank is used to document contamination resulting from the analytical process Method Detection Limit(MDL): The minimum concentration of a substance that can be measured and reported with99% confidence that the analyte concentration is greater than zero and is determined from analysis of a sample in a given matrix type containing the analyte Reportable Detection Limit(RDL): The lowest concentration that can be reliably achieved within specified limits of precision and accuracy during routine laboratory operating conditions For man} analytes the RDL analyte concentration is selected as the lowest non-zero standard in the calibration curve. While the RDL i_approximately 5 to 10 times the MDL,the RDL for each sample takes into account the sample volume/weight,extract'diaestate cleanup procedures and, if applicable, dry weight correction. Sample RDLs are highly matrix-dependent. Surrogate: An organic compound which is similar to : _e:a_-:alytc(s)in chemical composition and behavior in the analytical process, but which is not normally found in environ_r e- These compounds are spiked into all blanks, standards, and samples prior to analysis. Percent recoveries are cal:_ each surrogate. Validated by: Hanibal C.Tayeh,Ph.D. - - - = _:-.n authorized signature on the cover page.
