This Change Order consists of: AIA Document G701 CHANGE ORDER; 1 page dated 5 February 1997. These Specifications: 6 pages dated 5 February 1997. Two Drawings: SK-1 and SK-2 dated 5 February 1997. The work of this Change Order includes: New Storm Windows and frames(42) located above flat roofs (Classrooms,glass block clerestory windows) in the building's angled wings. New Fascia Boards. New Cricket(Roof). Removal and roofing over existing chimney; also modify venting of related gas water heater. Removal of existing carpet flooring. New Vinyl Composition Tile flooring. New suspended Acoustical Tile Ceilings. Temporary removal and re-mounting of existing items located on ceilings. The above descriptions are intended to give only a general outline of the work included in this Change Order. See all applicable Contract Documents and all Change Order documents for particulars. END OF SUMMARY OF WORK SECTION • CHANGE ORDER NO. 1 MASONRY CITY OF NORTHAMPTON, MA NORTHAMPTON PUBLIC SCHOOLS ROOF REPLACEMENTS JACKSON ST. ELEMENTARY SCHOOL PROJECT 334-A 5 February 1997 Page 2 of 6 GENERAL Masonry work in this Change Order consists generally of removal of indicated chimney to immediately below adjacent roof level, roofing over the removed chimney and modifying existing gas vent from water heater. RESTORE EXISTING BRICKWORK Repair/rebuild/patch existing brickwork where it is removed and/or damaged beyond the indicated removals. Use (only sound)existing face bricks taken from removed areas,where exposed, as far as possible and provide new matching SW type brick which may be required. Use SW type brick for any backup which may be required. All bricks thoroughly clean and wet immediately,before laying. Mortar: ASTM C270 Type N consisting of 1 Portland cement, 1 lime putty, 5-1/4 to 6 clean, sharp,damp sand (parts by volume). Brickwork match existing surrounding conditions; bricks,coursing, pattern,jointing, mortar color. All jointing: Press tight(tool),sharp, hard,smooth. Remove mortar spatter. Replace chipped, cracked and/or otherwise damaged brick units. Bricks wet when laid. Immediately remove mortar spatter and clean mortar and other stains. Leave all new and existing surfaces completely clean and undamaged at job's end. Use no acid in cleaning. Completely protect all work and all surrounding surfaces. Repair and/or remove any stains or damage to such surfaces caused by masonry work. Completely fill with mortar all bed joints, head joints and joints between wythes,solid piers and walls. Parge the backup with 1/4"to 3/8"of mortar before laying (and just in back of)face wythes. Do not slush head joints. ROOFING Extend new Elastic membrane roofing system to cover lower(remaining) portion of chimney. See SK-1,SK-2 and applicable parts of Specifications Section 7B-MEMBRANE ROOFING. Include all sheathing (3/4"exterior grade plywood)framing and blocking (pressure-treated) and tapered insulation; all contiguous with and blended to in-place new roofing system. GAS VENT from WATER HEATER Modify existing vent to exhaust through side wall of chimney, immediately below roof level, in a matching duct with weatherproof end cap at exterior. Slope new horizontal leg of duct±6" upward to exterior. End cap shall be a type which expels any infiltrated water. ELEMENTARY SCHOOL PROJECT 334-A 5 February 1997 Page 3 of 6 RESILIENT FLOOR TILE Vinyl Composition Tile: may be indicated VCT: Armstrong Standard Excelon Imperial Texture. 1/8"thick,through grained, 12"x 12"squares. Square pattern layout with uniform pattern direction throughout job. Existing ceramic block wall bases will remain. • • EDGE STRIPS Long lengths, continuous,tapered; at all following locations of all flooring in this Change Order: Exposed edges. Junctures of flooring which are not flush-finish with each other. Junctures of flooring with materials of other than this Change Order which are not flush with each other. Complimentary color,vinyl or rubber and with smooth, easy transactions between interfacing surfaces. INSTALLATION: ALL ITEMS Remove existing carpet flooring. Install direct to existing concrete surfaces, after removal of existing carpet flooring and subject to any preparation indicated below. Measure each space to receive finish flooring as a basis for furnishing and installing. Do not scale drawings. All accessories, edgings, etc.,as required for neat, complete, finished installations. Clean all receiving surfaces prior to installing finish flooring. Remove any existing applied substance which may affect application of new finish flooring. Apply only over clean,dry,smooth, level,true surfaces. Verify suitability of receiving surfaces prior to commencing work. At existing concrete surfaces, remove all existing carpet flooring and mastic down to firm concrete substrate material; recondition and prepare those surfaces as required to receive new finish flooring. Commencement of resilient flooring work indicates that this installer is satisfied with condition of construction to receive his work and that a completely satisfactory finished job shall be produced and any defects in the finished surfaces shall be corrected by this installer. Butt/scribe tightly to vertical surfaces, abutting materials and edging. Scribe neatly around obstructions. Produce neat joints, laid tight, even and straight. Final job neat,smooth,uniform,true and without wrinkles, buckles or blemishes of any sort. Unless indicated otherwise, extend flooring fully into toe spaces, open-bottom fixed furniture and full under standing items such as open-bottom equipment, convectors,water closets, radiators and other similar items (disconnect/reconnect/temporarily displace such items as required). Unless indicated otherwise, extend flooring of a main space into any adjoining closet, cabinet or similar minor space which has a continuous floor. Plan so that flooring pieces at opposite edges of spaces are of equal width (minimum 3") at space perimeters. Lay patteme square to space axes and continuous into adjoining spaces where same pattern extends visibly through spaces, unlesslindicated otherwise. Architect determine and approve all final layouts. Use underlayment/leveller at damaged, uneven, depressed,cracked or worn surfaces to receive finish flooring. "Silibond Masco/Latex Underlayment Cement." Adhesives recommended by manufacturer for particular applications: Use waterproof type. Asphalt eCnulsions and other non-waterproof types not acceptable. Apply primer, if recommended by flooring manufacturer,1rior, to application of adhesive. All components shall be"non-emitting"types. GENERAL,ALL ITEMS THIS SECTION • Upon completion of finish flooring installations, clean,wax(non-emitting) and buff, as applicable, all finish floors installed under this Change Order; methods and number of(wax) coats, etc., in accordance with flooring manufacturer's instructions. Remove all waste,excess materials,tools and equipment. Any damage to building surfaces as a result of resilient flooring installation is the responsibility of the resilient flooring installer; repair and/or replace au such damaged surfaces. Architect select colors and patterns from full color range as follows: Resilient floor tile: One color/pattern. Maintenance instructions: Submit manufacturer's written instructions for maintenance. Extra materials: Furnish for Owner's use the following minimum quantity of material. Do not use this material for punch list work,repairs. Suspended acoustical panel ceiling system consisting of 24"x 48"x 5/8"thick square edge, mineral fiber panels laid into suspended T grid system with 24"x 48" rectangular pattern grid. Wall angles or channels: Continuous at all abutting surfaces and tile terminations. Match T's. Hangar wire: Minimum 12 gauge galvanized steel wire. All connectors, accessories and additional hanger wires in sufficient number and spacing to support entire ceilings. INSTALLATION General removals of existing ceilings are not part of this Change Order. Remove only those existing (limited) partslitems which may be required for installation of new ceilings as indicated. Temporarily remove and re-install (in same locations on new ceiling surfaces) all existing items as follows; lighting fixtures,smoke and heat detectors,sensors and any other similar items(whether or not indicated)which may be attached to existing ceiling surfaces. Where lighting fixtures are suspended below new ceiling surfaces provide finished white escutcheons where suspension members penetrate new ceilings. Provide all required wiring, re-wiring, boxes, extensions and accessories for complete finished installations. Comply with all applicable Codes and Regulations. Permanently fasten hangers only to existing main structure above. Do not suspend from non-structural surfaces or from mechanical/electrical items. Space hanger wires maximum 4'-0"o.c. along all main Ts and all T ends. Additional hangers as required to support entire ceiling. Cut standard size tiles to fit job conditions. Install so that all spaces above suspended tile ceiling areas are entirely accessible by removal of tiles from room spaces below ceilings. Do not support lighting fixtures from ceiling system. Replace or clean any such components. Leave entire installations in perfect condition. At job's end, leave for Owner's use the following panels,in perfect condition.
