23.1 This visit was specifically to review the electrical work of Change Order No. 1. BJ and his maintenance people
had lately discovered deficiencies in the concealed electrical work which was covered up when ceiling tiles were
installed. Examples of the deficiencies were viewed in the Southwest-most Kindergarten room where BJ had
approximately half of the ceiling panels displaced from their grids (still laying on top of grids) so that concealed
spaces could be viewed.
Considerable discussion ensued re correction of the work. Per BJ, the Contractor's electrician never called for a
final inspection by GF. BJ had already engaged his own electrician to review and correct as required the entire
job since the Contractor had been notified by LPA±3 weeks ago,with no response. However, after consideration
between BJ, JC & LP, and since the job is still in the Construction guarantee period, it was their consensus that
the project Electrician be allowed to do the correctional work. BJ's electrician had not yet started the work nor had
any materials been delivered; therefore BJ, during the meeting, called him off.
It was the group's consensus that the situation be resolved as follows:
*The project electrician shall immediately commence correctional work(he did so today).
*He shall re-check the entire project, displacing and/or removing ceiling panels where necessary to reveal all
concealed electrical work and make any and all corrections according to a list of typical citations compiled by GF
(copies of which were handed to all parties today).
*He shall notify GF on a room by room basis for inspection before any work is covered up. GF will notify LP who
will be on call, if required.
*The work shall proceed rapidly and continuously to its completion and there shall not be any additional charges to
the City.

24.1 This visit was specifically to review progress in corrections to the electrical work of Change Order No.l 1.
Subsequent to the Meeting of 6 August 1997, GF had reviewed/pointed out the deficiencies, room by room, on site
with the project's electrical subcontractor who immediately went to work to make corrections.
24.2 Today's group commenced an inspection of the reportedly completed correctional work with results as follows:
*Room 2FS: Fixture suspensions and above-ceiling conduit ok. Need several fluorescent tubes and`7 fixture
lenses.
*Room 1 FA: Fixture needs box extension to ceiling level (fill gap).
*Room 24FS: Above-ceiling conduit OK but need 2 fluorescent tubes and 2 fixture lenses.
*Room 23SS: Needs 1 lens and support 1 fixture in triangular(plan) space.
*Room 22FA: 1 fixture needs knockout plug and support.
*Room 3SS (alcove): 1 fixture needs support.
*Two locations of punctured (from roofing connectors through metal deck)electrical conduit need repair/
replacement. Both locations are along south wall of corridor to the west of the buildings north entry hall.
24.3 At this point it became apparent that there was a pattern of incompleteness in the corrective work, th refore, the
inspection was terminated with the following conclusions, agreed to by all parties:
*Missing fluorescent tubes: Custodial staff will provide and install these.
*Missing lenses: Per SD, his efforts have indicated that new lenses are not available due to age of fixtures,
therefore it was the consensus that existing similar lenses will be switched from Room 6FS (Counsellor's Room)
and those replaced with similar new fixtures. I
*Completion of all work: SD agrees that the Custodial department will engage its own separate electrical
contractor to complete the corrective work and MDM Industries will be backcharged.
*Extra materials: MDM will provide 34 more ceiling panels (to complement 16 already provided)to mke total of
50 panels specified.
