Dear Victor,
The Board of Trustees has received your request of payment. In April of 2001 you notified the
Management Office of water damage in Dorothy Charsky's Unit No. 5-101. At that time, you were
advised of the $5000.00 deductible, it was suggested you contact your insurance company.
The River Run Condominium Association was never notified of any damages that exceeded the
$5000.00 deductible, nor was the/Association's Insurance Company. If proper measures were taken at
that time, an Adjuster would have been required to assess the damage and act accordingly.
Not only was this Insurance issue addressed improperly, but you also took it upon yourself to
compromise the structural integrity of the unit by splicing studs and supplying temporary support to
ceilings and other structures. Please provide the River Run Condominium Association Office with the
permits granted for the completion of this work.
You also made mention of lying into" a firewall. Again, please provide the River Run Condominium
Association Office with the permits granted for the completion of this work.
Furthermore, The River Run Condominium Association only hires contractors when there is a specific
scope of work agreed upon and a professional contract signed by the Board of Trustees. Also required
is proof of Insurance and most definitely a valid contractors license. Not one of these requirements was
met prior to the work preformed in your mother's unit.
Finally, I must inform you that the River Run Condominium Board of Trustees has denied your request
for payment of this work. The Northampton Building Inspector and The Northampton Fire Department
will be provided a copy of you letter for their files.

Notice of Violation - Failure to Maintain a
Fire Alarm System

Dear Ms. Hopp:
I am writing to inform you of numerous violations of Massachusetts's General
Law Chapter 148, Section 27A for failure to maintain a fire alarm system. This
violation is highlighted by the repeated failure of the fire alarm systems located at
River Run, 80 Damon Road, Northampton, MA to properly transmit alarms to our
dispatch center.
During the most recent test of each system, four buildings failed to transmit any
alarm and the remaining four transmitted a signal at an average time in excess of
that allowed by the Massachusetts State Building Code. In addition, this system
has failed previously. Based upon this situation and your vendors repeated
failure to obtain required work permits coupled with their demonstrated inability to
properly transmit alarm signals, the following action must immediately be
undertaken:
1. Permits for all work performed within the previous twelve months must be
filed retroactively with both the Northampton Fire and Building
Departments.

2. Fire alarm work permits need to be pulled for any future work done on
these systems.
3. These systems need to be connected directly to the Northampton Alarm
receiving System or to a different approved central station.
4. Emergency access key boxes, yellow engraved key tags and red indicator
strobe lights need to be installed at each building per City Ordinance.
5. The following items need to be installed per the Northampton Fire
Department Installation Guide:
❑ A color coded graphic map of each structure next to each
fire alarm panel
❑ Signage directing personnel to each fire alarm control panel
❑ Signage numbering on each detector
❑ Signage on each digital communicator and on each Fire
Alarm Control Panel
Please contact me with any questions at (413) 587-1039, I would request a
written response with a commitment to address the five points listed above no
later than Friday March 26, 2004. This letter should to the extent possible
indicate timeframes and contractors that will be responsible to perform this work.

immediately contact the Northampton Fire Department and Building Inspector, Tony Patillo,
relating to the unlawful dialer replacement work and the failure to obtain permits and that you
report to us immediately that you have rectified these matters.
Of course, ADT will be responsible or any fines levied against the Condominium Association
relating to your company's actions or inactions.
