I, Anthony Patillo, Building Commissioner for the City of Northampton, on oath
depose and say that the following is an accurate account of an inspection which
took place at 80 Damon Road, Apartment #_4109 , Northampton,
Massachusetts, on October 22 , 2008
1. On October_22_,2008, I was asked by Northampton Police
Department to inspect the Apt #_4109_, to determine if there were
health and safety concerns.
2. I inspected Apt#_4109_ and found the following building Code violations
on the premises:
• ;Two electrical outlets had be tampered with by removing the
outlets receptacles and cover plates leaving bare exposed
wiring, (photos attached)
• Several receptacles had been damaged by blunt force exposing
bare electrical wires.(photos attached);
• Two receptacles had been opened and altered leaving bare
electrical wire exposed.(photos attached)
• ;All of above work was not performed by a licensed electricician
nor were electrical permits applied for
• ;Extension cords had been chained together stretching from
front of apartment to rear bedroom.
• ;
3. I reported the above violations to the Board of Health on October
22 ,2008

I, William Millin, Fire Inspector for the City of Northampton, on oath depose and
say that the following is an accurate account of an inspection, which took place at
80 Damon Road, Building 4 Apartment # 4109, Northampton, Massachusetts, on
October 22, 2008
1. On October 22,2008, I was asked by Deputy Chief William Hurley to
inspect the Apt #4109, to determine if there were health and safety
concerns.
2. I inspected Apt # 4109 and found the following Fire Code violations on the
premises:
• 527 CMR 1.06(2) - Failure to maintain a fire protection system.
(Smoke detector disabled and the base appeared to be burnt)
• 527 CMR 31 — Failure to install and maintain a Carbon
Monoxide detector.
• MGL 148 sec 23 — Improper storage of flammable liquids. (Gas
can and other various flammable and combustible liquids on the
floor in the living room.)
• 527 CMR 1.06(1)(a) — Dangerous conditions which are liable to
cause or contribute to the spread of fire or explosion in or on
sad premises, building or structure or endanger the occupants
thereof. (Careless disposal of cigarettes, Smoking in bed, and
burnt matches taped to the wall.)
• Unknown other Violations-Almost all cabinets, draws and
closets were padlocked.
3. I reported the above violations to the Board of Health on October 22, 2008
4. For the above violations there are attached pictures.
