Section 110.R5.1.2
Homeowner: person(s)who own a parcel of land on which he/she resides or intends to reside,
on which there is, or is intended to be, a one or two family dwelling, attached or detached
structures accessory to such use and/or farm structures. A person who constructs more than one
home in a two-year period shall not be considered a homeowner.
Section IIO.R5.1.3.1
Any homeowner performing work for which a building permit is required shall be exempt from
the licensing provisions of 780 CMR 110.R5, provided that if a homeowner engages a person(s)
for hire to do such work, then such homeowner shall act as supervisor.
Such homeowner shall submit to the Building Official,on a form acceptable to the Building
Official, that he/she shall be responsible for all such work performed under the building permit.
As acting Construction Supervisor your presence on the job site will be required from time to
time, during and upon completion of the work for which this permit is issued.
Also be advised that with reference to Chapter 152(Workers' Compensation)and Chapter 153

Pursuant to your request,IA Engineers has performed a limited structural evaluation of the roof framing at the above
referenced site to determine if the roof has adequate capacity to support the proposed Solar panels.Our analysis was based
on the framing Information and configurations provided by Devlin Contracting and Maintenance Inc..It is our understanding
that the structural components of the existing roof framing are in good condition.It Is further understood that all existing
connections between the various roof framing members are adequate to must the current loading conditions and behave in
the manner that a typical rafters and ceiling tie system is intended to behave prior to installation of the solar panels.
Results:
Roof 1–adequate to support the proposed solar panels
The spacing of the solar standoffs should be kept at 48"o.c..In addition,It is important that the solar standoffs locations be
staggered between adjacent rails so that no single rafter supports more load than under the existing conditions.

General Terms&Conditions for solar systems Installation
performed by Insight Solar:
1.Installations must be completed in accordance with all laws,codes and other requirements applicable under federal,state
and local authority(eg.permits,inspections)
2.We will require the necessary client information to proceed with mandatory Utility and State applications.
3.Insight Solar and its staff,including it's subcontractors,will require access to your property to proceed with initial project
assessment.construction inspection and required maintenance.The client will be contacted prior to any visits,and will reed to
arrange property access allowing us to fulfill its portion of the agreement.We will require all necessary furan filled and full
client cooperation to complete its part of the contract.
4.We recommend the client to seek the appropriate consultation concerning any tax liabilities that may be associated with the
receipt of the rebate.
5.Insight Solar,its parents,subsidiaries,affiliates,officers,employees,contractors and agents:
Are not responsible for any tax liabilities that a client may incur.
Assume no responsibility for the performance of the equipment or equipment warranty,
-Do not endorse,guarantee or warrant any particular brand,manufacturer or third party services.
6.The client hereby agrees to indemnify,defend and hold harmless, Imight Solar, its parents,subsidiaries,affiliates, officers,
employees, contractors and agents from any and all liability associated with this program/project except for the labor
warranties provided.

7. No amount of energy savings, energy production or income generation are guaranteed as a result of the purchase and
installation of eligible equipment.
B. We are responsible for the application processes, from initiation through completion unless mentioned otherwise by a
representative of Insight Solar.We may from time to time request your assistance in the application process.
9.It is the responsibility of Insight Solar to ensure that all appropriate permits(e.g.local,state)have been obtained including
ensuring that Deed Restrictions or Home Owner Association Restrictions;do not prohibit the installation of the proposed PV
system.
10.Federal,State,Local, Utility program guidelines and rebate levels,if applicable are subject to change without notice,and
may be terminated by executive,legislative or regulatory action at arry time.Utility rates are also subject to change.While we
strive to expedite all application processes.Insight Solar is cannot be held accountable for any loss of rebate,income,savings
or delays resulting from any such situation.
11.Prior to installation,Insight Solar reserves the right to conduct an additional site visit/audit to verify that all satellite or
physical calculations were accurate.Materials offered are subject to change without notice.Quality and efficiency offered will
remain equivalent or better.
12.All payments must be made according to the scheduled agreement.Personal Check,Wire Transfer,and Bankers Draft are all
acceptable methods of payment.All goods remain the property of the Insight Solar until final payment has been completed.
Payments outstanding beyond their respective due dates will accrue interest at the rate of 1.5%per month on the outstanding
balance,until the balance is paid in full and final settlement has been reached.
13.Insight Solar does not,will not,and cannot hold responsibilities in relation to any form of damage caused by accumulated
snow and/or ice and/or hail on or around the solar array.
14.The timeline proposed in this offer cannot be guaranteed by Insight Solar and Insight Solar carat be held accountable for
any delays resulting from situations not under the control of Insight Solar(rain,snow,application approval delays etc.)
15. Insight Solar will not sell, share, or ren[ your personal information to any third party, or use your e-mail address for
unsolicited mail.All e-mail messages sent by Insight Solar will pertain solely to the provision of agreed services and products.
Any information on pricing and procedures will remain confidential between the client and Insight Solar and are not to be
shared with other companies.
16.Warranty or maintenance obligations information maybe sold to or transferred to a third party at anytime.
17.By signing electronically,and upon doing so,client agrees to all program requirements including all terms and conditions of
Insight Solar and both parties are executing this offer in goad faith.

ADDENDUM A: WORKMANSHIP
WARRANTY
1.This 10-year workmanship warranty applies only to systems installed by Insight Solar and includes all parts and labor for
workmanship errors associated with repairing the
system.Requests for service will be investigated as soon as possible by Insight Solar
and findings will be reported to the customer. The 10-year term begins on the date system passes final inspection by
permitting authority.
2.The warranty also covers customer's roof against leaks resulting from solar panel installation for a period of 5 years from
final inspection,Insight Solar installations adhere to the highest industry standards for waterproofing and durability to ensure
a clean and leak-free installation.Customer agrees to contact Insight Solar at the first sign of a roof leak. [might Solar will
analyze the problem to determine the cause and report to the customer.If Insight Solar finds the intrusion resulted from our
installation,we will then perform the repairs needed to fix the leak.Any other repairs shall be subject to mutual agreement.
Leak warranty does not cover damages resulting from roof leaks.
3.Warranty does not cover damages due to accidental, incidental,or intentional acts,including but not limned to acts of God
(eg., earthquakes, fire, hail, wind storms etc.) or damage due to actions or inactions by the owner or other persons. This
warranty does not constitute or in any way impose a duty on Insight Solar to provide pest control services, devices, or
improvement materials designed to prevent or remove pests(such as pigeons,rodents,insects,etc.)from solar arrays.
