SH to contact HPMG. V 12.1 Not resolved yet TCI to contact Gas Co. V 12.2 Gas meters racks located about 10' down north wall and hang into abutters private property. SH also concerned for damage from haulers and snow plows, requests relocation. SH requests Northampton Coffee's switch- over occur last for least impact. TCI will follow up to confirm that it is OK to hang meters over private property, request relocation, and determine how much of this relocation expense the Gas Co. pays for, who provides bollards to protect meters, & what is plumber increase expense is for additional work. V Page 1 12.3 Gas Co. is relocating the meters as an emergency need, no charge. EWS will be here Wednesday, complete their work by end of week, TCI to schedule gas inspection for Monday. Location of existing compressor for coffee shop AC and new gas meter protection bollards need to be surveyed for easement document. SH inquired why this move outside was unexpected. SH inquired if meters could remain, work performed around them in place. TCI explained the current location is an existing hazard to have in the basement and presenting hazard to construction work. SH asked for assurance that no one will be without gas over weekend. Proceed with relocation as directed by Gas Co. TCI to measure bollard and compressor locations, send info to Eaton. TCI will determine swap times and let SH know, probably a few hours' duration, SH will give notice to tenants to allow permission to enable subs and Gas Co in to relight pilots. V 12.4 Gas Company indicates to TCI that Gas Co. does not have an easement over abutter's property. SH feels Gas Co indicated to her they did. Further research is needed. SH to pursue.V 12.5 NO EASEMENT EXISTS. Best current understanding is Gas Co considered Florida Ave a public street and did not need to get permission from, nor give notice to, private property owner. SH reports MPI is negotiating with abutter for an easement. Gas Co. has not shown up three times now, delaying project about a week. SH states MPI is quite upset with Gas Co unresponsiveness, very concerned for delay this causes and subsequent increased costs. TCI, to promote good will and minimize delay, is negotiating with abutter to address concerns for Gas Co patch. TCI asks SH as owner to try to motivate Gas Co since TCI as contractor doesn't have leverage. SH will contact Gas Co, contact person appears to be Mike Angoletti. Meter relocation will proceed while easement is prepared. SH, TCI 12.6 Meters are moved! Two more bollards need to be installed. TCI provided as-built measurements to SH, TCI to call Gas Co for last two bollards. Driveway proposal to be reviewed with abutter early next week, work scheduled for 19th.meters moved TCI to contact Gas Co to have bollards installed before driveway work. TCI 33 SH inquired if storefronts could go in earlier, indicated willingness to pay to repair damage for installation before brickwork complete. MF will review how early storefronts could go in and report next week. V 33.1 MF indicates storefronts could go in as early as 1 November.TCI confirm with Lizottes V 33.2 MF confirms, and cautions that installation is still subject to delays from gas meters, weather, city inspections; so not a guarantee. SH will sent notice to tenants that project is pursuing the possibility of accelerating storefront installation perhaps a few weeks. V 33.3 Gas meter delay means expedited approach might result in storefront installation as originally scheduled. SH to notify tenants of updated schedule. V Page 2 33.4 SH inquired about schedule of outstanding first floor interior work after new storefronts are installed. TCI reviewed work related to new ceilings, patching ceiling and walls, painted, floor installation and finish. Roughly another 1.5 weeks' work. SH asked if this time period could be shortened citing tenant business impact. LG indicates possibly. None.09-Oct-12 34 CB confirms there is no insulation in attic, SH requests quote to insulate attic MF to arrange V 34.1 CB confirms there is no insulation under roof membrane either TCI to contact insulator for quote. Include flash spray around ceiling penetrations to aid in vapor barrier management, R-40 blown-in cellulose, leave sufficient extra mounded 4' from front wall to be distributed by TCI after wall framing installed. V 34.2 Quote provided. PF asked for confirmation that, where fiberglass batt is used to fill gaps in lath and plaster original ceiling, the area will be flash sprayed to maintain air-barrier continuity at that level; if there are recessed fixtures that penetrate the original ceiling that appropriate insulated boxes be provided. Confirmation that dropped ceiling is not the air or vapor barrier, and insulation is to be installed above upper original ceiling. SH has several weeks' time to review budget and decide.V 34.3 SH indicates desire to do work, is following up with rest of family SH intends a decision by end of this week. SH 43 During field observation, RH noted top of north wall is potentially soft brick & mortar without much weight to hold all in place, urges use of care when installing skewed saddle to receive LVL roof beam. Install anchors by power-drilling, if hard brick encountered then hammer-drill only as necessary. CB 45 CB anticipates pouring concrete beam late next week, wants to know who determines how far into existing brick foundation to remove. RH to continue periodic field observations, will determine depth of removal or repair as work progresses to that point. V 45.1 RH & CB reviewed brick foundation removal in two locations not involving gas meters. Bricks removed by hand to a depth of 16"+/- below which brick can be removed with hammer. Agreement to remove 2-3 more courses to sound brick. None, if remaining foundation wall is similar. V 45.2 Foundation brick at south end are sound at depth of 16", foundation brick at north end still questionable, remove another course or 2; foundation brick in middle not removed yet as gas meters have not been removed. RH suggested to use hammer drilling for grouted anchors to test soundness of remaining brick. V Page 3 45.3 Anchors installed. RH, CB & conc. sub observe that wall appears to be in two parts. Outer portion of foundation wall is stiff, with few voids. Inner portion is softer, looser, more voids & w/out much connection to outer portion . It appears the inner portion was added at a later time than the outer portion, consistent with a what might have been a previous effort to shore up the original wall.. None 09-Oct-12 46 Discussion of how to determine top of concrete grade beam in relation to floors, which are not level. PF notes new sidewalk warps to meet new alcoves which can vary in slope if needed but cannot exceed 1:12 pitch. CB will survey entry door threshold heights to determine top of new grade beam.V 46.1 Survey reveals that floors are at different levels between storefronts, and sloped within storefronts. General descent from south to north, more pronounced at south end and tapers to north end. Agreement to use a low- point around column C1. None V 46.2 CB provided further spot elevations of existing sidewalk to PF to aid resolution of how to accommodate varying floor heights and establish T.O. conc. grade beam. PF to plot information, meet with CB and MF to review.PF,CB,MF 46.3 All done, CB proceeding. MF points out we still need info from storefront supplier regarding adjustability of new door thresholds. PF notes the opportunity to level floor somewhat when new basement post is installed. MF to follow up.MF 50 PF reviewed shop drawing log to confirm submittals. TCI submitted additional submittals. TCI noted need for immediate review of outstanding steel shop drawings. PF to provide sketch (item 38.1), sub to resubmit to TCI, TCI, RH and PF to review as soon as possible. V 50.1 Steel fabricator requests confirmation of layout dimensions including vertical before resubmitting. Re: Item 46.2, provide tomorrow Wednesday if possible.V 50.2 CB has provided requested info, fabricator proceeding. After discussion, agreement to have fabricator submit revised shop drawings for record. SH asked for confirmation that this shop drawing review is not a delay, TCI concurred, steel installation scheduled to begin next week if concrete work completed this week. PF inquired if conc. testing is required; after discussion, agreement that RH will confirm if Code requires for this project. TCI indicates they intend to use 4000 psi, 3000 is spec'd. Consensus that this safety- factor is sufficient, skip test if not Code required. Submit revised steel shop drawing in PDF to RH and PF. RH to report on Code requirement for conc. testing. RH reports conc. testing is required for beam, but not for footings. MF,RH,PF Page 4 53 CB reports: City gas inspection scheduled for 2:30 Wednesday, switch-over tentatively scheduled for sometime Thursday at earliest; mason patched around new gas line penetration in north wall; need to get brick ordered soon (PF waiting for samples, to be delivered Wednesday am); foundation work is stopped pending meter removal; staging cleaned to minimize falling grit. Optimistic schedule: meters moved Thursday, remaining foundation removal to Friday or Monday, concrete forms on Monday or Tuesday, rebar installation remainder of week, pour conc. bond beam end of week or early next. SH to notify tenants of updated schedule. V 53.1 Gas inspection done last Wednesday, but Gas Co has not relocated meters. Existing meters hopefully will be removed Thur. this week. All noted that Gas Co information is proving unreliable. Gas Co will mount new meters outside building; once actually done SH will provide 24hr notice to tenants; EWS will switch over from old meters to new meters; Gas Co will collect old meters 09-Oct-12 55 TCI submitted more shop drawings. MF & PF to meet after to confirm final wall section. PF to review submittals per outcome. V 55.1 MF & PF met, wall assembly is as drawn. PF processed some of submittals, others need resolution of T.O. conc. grade beam first. PF to process remaining submittals. V 55.2 PF noted that he doesn't have paperwork for masonry samples submitted last week.TCI to provide.MF 56 RH observes saddles for roof beam fabricated without incorporating corrections to shop drawing submittal. Still awaiting steel resubmittal. TCI to return saddles for modification. TCI 61 PF inquired of SH status of building signage and awnings; noting that in order to proceed we need to get fabricators under contract. SH to contact PF to set up meeting. SH 61.1 SH & PF to meet after this meeting PF to begin drawings for business tenant signage frames, first floor awnings, and building directory for quotes, permits, and building integration during construction. PF 61.2 PF reports information has been provided to Porcupine Signs, meeting tomorrow Wednesday to review with goal to provide pricing for approval. PF to follow up.PF 64 Due to differential (& lower) elevations of existing second floor, decision to lower beam 1 brick course to accommodate 3" steel lateral post brace angles . After meeting, MF & RH worked out how to hide down-leg of angle in wall, no longer need to lower beam 1 brick course. TCI to minimally notch attic support columns as needed to set angle into wall/ceiling corner, beam elevation remains as is. 09-Oct-12 65 Due to varying elevations of existing first floor, need to revise 6/S1 at second floor.RH & CB to review after meeting. V 65.1 Detail in CDs is confirmed. None 09-Oct-12 Page 5 68 CB confirmed that east face of new conc. grade beam will be up to approx. 6" east of brick foundation wall below in places. CB inquired if full 24" depth of grade beam is needed if unnecessary to remove foundation brick to that depth. RH preliminarily indicated 16" likely sufficient depth. RH to review and confirm with CB V 68.1 RH confirms 16" depth is sufficient. CB notes typical depth will be 19-20"None 09-Oct-12 69 CB inquired of how to integrate new conc. grade beam with existing conc. foundation patch at north end. RH indicated remove sufficient depth from top of patch to ensure minimum 6-8" new concrete at top. None V 69.1 CB indicates actual depth will be about 10". MF suggested stay at weekly for now, go to biweekly perhaps in a few weeks. Agreed. None V 73.1 With most major obstacles identified and resolved at this point, agreement to go to meeting every other week. PF still will visit site once a week or more if needed, RH will visit as needed. None 09-Oct-12 74 TCI submitted more shop drawings. PF to process.PF 74.1 PF returned some processed shop drawings, requested a comparison of TCI submittal log with PFRA log to confirm coordination. Brick sample panel for north wall approved. PF and Jacquelyn to compare PF,JB NEW BUSINESS 77 CB reports: gas meters relocated; remaining unsound foundation brick is removed; concrete mass at north end of foundation lowered; benchmark work is completed; attic and roof structure modification begun. Concrete piers and basement footings being poured today, formwork and rebar setting begins soon after; concrete grade beam to be poured by Friday; steel installation to begin on Monday; upper wall framing begins this week. None 09-Oct-12 78 Chuck Miller inspected excavation, noted need for electrical ground connection to rebar. TCI will be add this to other improvements being required to existing building electrical ground. CB to coordinate with formwork installation. Due to need to cut gypbd in order to get attic beams in and still have maneuvering room, at first temporary walls in third floor apartment will be taped poly, once beam in place, then plywood temporary wall installed. CB will notify Northampton Coffee of temporary wall installation, probably Tuesday next week maybe the next day. SH to confirm notifiction w/ tenant - same beige background, tenant paints at commercial, McCarthy paint residential 04-Sep-12 6 CB reviewed roof support post details, RH confirms attachment at bottom of post on plate on existing 6x6 beam on masonry wall; splicing the post, stagger by a foot, close to floor or ceiling. CB & RH will review in field after meeting 04-Sep-12 7 CB reviewed roof support beam bearing details. RH confirms not splicing the beam, beams begin and end on centerline of post, suggests lapping 2x6 down sides for lateral bracing and attachement; no bearing plate/saddle needed. None 04-Sep-12 Page 7 8 CB points out there is barely enough room above third floor false ceiling and under attic ceiling joists for beams, both in terms of depth of beam and to accomdate out-of-level condition of existing building. Conclusion to keep as designed, slope beam to existing conditions. RH will determine allowable amount to be trimmed if/when necessary; confirms no saddle shop drawing submittal required. V 8.1 CB reports ceiling in kitchen is higher than in other rooms, conclusion to allow approx. 1"x 6" drop in ceiling. None 11-Sep-12 9 CB indicates staging installation is scheduled to occur on Tuesday and Wednesday next week. MF notes this may slide a day or two, depending on foundation inspection requirements. Ongoing this week. V 9.1 4 Sep 12 - TCI will request larger outriggers to provide more maneuvering room. MF to arrange V 9.2 Larger outriggers pushed scaffolding further onto the sidewalk impinging on street furninture, etc. Cribbing provided in trench instead 11-Sep-12 10 TCI anticipates sufficient sidewalk removal Friday to provide three foundation inspection pits: 2'deep, 2' wide, and 3-5' long. CB will notify RH & PF when ready, RH will inspect and if needed indicate locations for further excavation and inspection. Trench to 8' deep, foundation wall removeal to 24" down. 04-Sep-12 11 MF explains once foundation trench is complete in an area, trench will be partially filled w/ membrane installed at an angle with excess collected above fill placed on top of membrane, to povide base for scaffolding. TCI to sumbit and PF to review membrane shop drawing immediately. CB will contact Beaulieu to arrange for temporary power from basement 04-Sep-12 15 EWS is removing baseboard heat at third floor apartment Wednesday. CB has reviewed with tenant. CB to confirm if this removes heating from entire apartment or just front rooms. V 15.1 Unclear where there is heat in apt. SH to survey what rooms are effected, provide plug-in heaters as needed V 15.2 Second floor apartment has electric heaters, which are still operational in unaffected rooms. Third floor apartment has electric heater in just one back bedroom, inoperable hot water baseboard elsewhere. SH will provide heaters to third floor apartment soon.V 15.3 Heaters provided last week. None 02-Oct-12 16 CB reports the dumpster will be placed in parking spaces in center of building day after scaffolding is complete. None V 16.1 Due to more extensive trenching, scaffolding delayed a week, dumpster will arrive end of Friday or first thing Monday If Friday delivery, CB to cover so it is not used by others over weekend.18-Sep-12 Page 8 17 LG noted to SH that third floor tenant contacted TCI for an update, CB is in touch with him each day. None 18 SH announced that second floor apartment is rented as of 1 Sept. TCI to install temporary wall before move-in. V 18.1 SH indicates tenant will move in mid-Sept. SH will confirm actual dates V 18.2 SH reports tenant begins move-in on 18th, asks for a few days before to clean.TCI to vacate on 13th so MPI can prep apt. 18-Sep-12 19 LG asked PF to review draft submittal schedule to confirm items needed.PF to review, TCI will send V 19.1 PF did review draft, which was for confirmation of required submittals, no dates included. Ryan asked to be included in the distribution. TCI to submit. 25-Sep-12 20 PF reported that price quoted to Beaulieu for light fixture "A" is well over estimate provided by distributor in Boston. PF is attempting to contact distributor. PF suggested we may wish to consider an alternative approach, particularly if it coordinates with business tenants' requests for vertical signage above awnings. SH indicated McCarthys are not enthusiastic about banners. PF to follow up with distributor. V 20.1 PF contacted CT distributor instead of Boston distributor, who offered a substitute. PF also contacted a third distributor to get a second quote for originally chosen fixture, distributor submitted quote that fits budget, indicating an example was to be installed imminently in West Hartford we could visit. PF to follow up, visit site. V 20.2 Fixture installation in Hartford not soon enough, distributor provided LED lamped sample for review. Fixture approved PF to follow up with lamping recommendation. V 20.3 PF indicates can't justify the additional expense for the LED bulbs, recommends proceeding with metal halide. Agreed. PF to provide TCI w/ specs. V 20.4 PF returns sample to TCI field office for distributor to pick up. Scott Dubey has CB's cellphone number to confirm arrangements. None 25-Sep-12 21 SH requested that when soil is removed for foundation inspection, samples be taken for submittal to Umass extension service for analysis to determine if there is something in soil that is killing trees in sidewalk. SH will coordinate w/ Bill McCarthy to be available on Friday with three buckets to receive samples. 11-Sep-12 22 LG requests that distrubution include subs LG to provide list, PF to include in distribution 11-Sep-12 Page 9 23 CB reports storefronts anticipated to be removed by end of day; tenant in third floor is OK with work beginning at 7am with notice; roof beam installation begins tomorrow; trenching along foundation begins tomorrow weather permitting. SH to confirm Emily is sole source of Northampton Coffee direction. SH 23.1 Makunda has returned early. SH confirms Emily remains contact person and sole source of decisions. None 11-Sep-12 24 Observation Holes: Brick appears in rough to poor condition to a depth of perhaps 9 courses down. RH requests deeper trench to confirm condition of top 24" of existing brick. MF indicates trench to 36" deep, will use excavation equipment to accomplish. 11-Sep-12 25 Observation Holes: top of foundation wall appears to have bowed in 4" +/- towards center, top of foundation wall will not be under new bond beam. RH indicates need for piers to be poured exterior of existing foundation to below frost; agreement that north and south ones will be smaller than intermediate ones. RH will email dimensions for footings by Thursday morning, record sketch to follow within a few days. TCI to excavate for piers, clean brick and pin, provide forms to be buried based on RH info. Pour piers Thursday, backfill on Friday. V 25.1 Trench dug to depth of 4-6' down to ensure new piers bear on undisturbed soil, approx. 18- 24" deep flowable fill provided between pier formwork. RH determined pins from piers into brick are not needed, but up into grade beam are, 2 at ends and 3-4 at intermediates. Due to cantilever of grade beam east of foundation wall, #3 ties 12" O.C. are needed in beam. RH will provide sketch once depth of foundation brick repair is known. V 25.2 Dimensions phoned to TCI on Wednesday. RH record sketches, including pinning detail between pier and grade beam, as well as additional ties within grade beam, are pending. RH to provide sketches V 25.3 Sketches provided. After discussion over sequencing, RH stated that if bond beam is done in two pours, he wants to know where cold joint will be. RH also stated he will prepare record set of his documents incorporating revisions. CB to notify RH, if two pours. CB 25.4 Decision to use two pours, first includes basement footings while still able to access from front. None 02-Oct-12 26 CB inquired of location of new basement footings. RH confirmed under corner of flat and sloped framing in first floor alcoves. RH and CB to review after meeting.V 26.1 CB would like to reconsider one location RH and CB to review after meeting. 18-Sep-12 27 CB and RH reviewed threaded rod and epoxy requirements at saddles and existing brick foundation wall. RH confirmed use of Hilti HY20 with screen tubes. None. Hilti nolonger makes hy20 uses hy70 RH ok 11-Sep-12 Page 10 28 MF reports foundation membrane arrives Thursday. TCI to confirm with Northampton DPW exact sub-sidewalk specification requirements.V 28.1 Due to cribbing, installation of membrane is delayed. MF confirms continue flowable fill between piers to underside of membrane. Keep membrane down to allow underslab drainage above membrande.V 28.2 Conclusion to provide membrane approx. a foot or so below sidewalk.None 02-Oct-12 29 LG submitted draft pay req. SH concurs. MF to arrange 18-Sep-12 38 RH requested PDF copies of steel shop drawings on jobsite MF to arrange V 38.1 RH reviewed, resubmit drawings. RH reports conflict with bolt pattern at post to beam connections and closure plates in Architectural detail. PF indicates issue resolves if closure plates are only 3" deep, sufficient to close around Azek panel, will issue sketch ASAP. Also observed that intermediate cast iron posts were not anchored to stone bases, as stone bases twisted and/or tilted, post bases were skidding off. None 25-Sep-12 58 CB reported that a steel channel was discovered lagged to seconed floor wood beam between B1 and C1, a second steel channel was discovered lagged to beams from C1 to E1. Shorter beam had bowed. Speculation that these were an earlier attempted repair, likely explanation for reduced wall damage between B1 & E1 compared to between A1 & B1, where no steel channel was attached to the beam.
