Next Project Meeting Is Tuesday, June 5th. 8:30 am – Retail Canopy Meeting All subcontractors with work ongoing or started/completed within one week of this meeting are required to attend. CURRENT PROJECT STATUS Excavate retaining wall #2. Lamp and ballast removal – complete. Sheath Phase II canopy. Demo walls to access additional abatement areas – complete. SCHEDULE MILESTONES Phase III Steel TBD Phase I Sidewalks TBD HARD HATS ARE REQUIRED FOR ALL CONSTRUCTION PERSONNEL AND SITE VISITORS. SAFETY cm&b Safety Policies: Written safety manual and applicable MSDS information to be provided to project superintendent prior to start of work and/or delivery of materials. A SAFETY VIOLATION PENALTY POLICY IS IN EFFECT ON THIS PROJECT. ANY VIOLATION OF O.S.H.A. SAFETY REQUIREMENTS IS SUBJECT TO THE FOLLOWING ACTIONS: FIRST VIOLATION – VERBAL NOTICE. EMPLOYEE RETRAINING REQUIRED. SECOND VIOLATION – WRITTEN NOTICE. EMPLOYEE RETRAINING REQUIRED. THIRD VIOLATION – $100 FINE TO THE COMPANY OF THE EMPLOYEE RESPONSIBLE. RETRAINING REQUIRED. FOURTH VIOLATION – EMPLOYEE PERMANENTLY REMOVED FROM SITE. Violations Assessed Within The Last Week: None. Hardhats, shirts, long pants and work boots are required by all construction personnel through project completion. All Subcontractors are required to hold weekly employee safety meetings (“Tool Box Talks”). Foreman must submit a copy of topic discussed, with signatures of employees in attendance, to cm&b superintendent by Friday, 12:00 noon each week. Weekly Safety Discussion: SLIPS AND FALLS Injury, accident, damage, theft or any other incident must be reported immediately to cm&b superintendent. NO EXCEPTIONS. First Aid station located in cm&b field office. cm&b Loss Control Guidelines. GENERAL ITEMS 3/13 Staples Pre – Construction / Design Meeting. Remove RTU’s and patch roof. Demise wall locations ok. Storefront work (canopy by Big Y) – ok with approved. Staples / Big Y canopy option #3 ok – Correction: Revise Option “A” dated 3/13/07 agreed by Staples. Staples changed storefront glass to spandel. Will need City approval. Retail Canopy upgrade is 18 week schedule. Staples to apply for signage permit with Building Dept. Big Y has no involvement with this item. GENERAL ITEMS (continued) Staples Pre – Construction / Design Meeting. Canopy lighting designed to be fed out of Table and Vine space. Big Y to look into alternate retail space for power. Staples agreed canopy construction in front of pace is acceptable. Staples fixtures allow for product 14” high. No rack sprinklers will be required. Big Y anticipated space turnover date will be July 1. Big Y / Staples to define interior demolition for walk in coolers and refrigeration systems removal. Staples design not to use existing Table and Vine loading dock. Any change would need Big Y and Town approval. Big Y and Staples agreed to coordinate new sprinkler work would be done in harmony with Staples contractors. Lease questions and comments from Frank Coe: We will remove and cap all the HVAC units on the existing Table and Vine We will remove all Table and Vine store equipment and refrigerated cases. We will abate all tile and mastic that is suspect. We will remove the free standing cooler box in the back storage room. We will reclaim all refrigerant (HVAC and refrigerated cases) We will not remove ceilings All roof work to fall under the roofer of record (Morris Roofing) We will present the revised storefront /façade as agreed too but if we get rejected by the City we will need to have Staples pursue this approval as Big Y will not go back through P&Z for approval. (specifically the opaque glazing) Staples will need to get City/Fire Department approval on the proposed loading dock location as it pertains to access and traffic flow around the retails. As we progress closer to turnover and mobilization of your contractor we will need to get a certificate of insurance from Fortunado which names Big Y as additionally insured. Staples will be responsible for their own signage approval HFP (sprinkler contractor) will work in harmony with your general contractor on the installation of the sprinkler system and it pertains to schedule and sequence of the project. Turn over of the space will be on or about 7/1/07 3/27 Staples needs approval for temporary sign banner and coordinate location with ongoing construction activities. PERMITS SUBMITTALS Retail Façade Pending Submittals:   DESIGN Current drawing rider is attached.  All subcontractors please verify current drawings for field and office use. All cost issues must be documented and quantified with labor and material breakdowns.  Updated drawings are available through Service Point, Woburn, MA (781) 933-8700 phone / 781) 935-3926 fax. 3/27 Retail Canopy light feeds will be redesigned to utility room behind Ken’s Eyewear. 5/8 Final location will be coordinated with main electrical service above General Cleaner’s space. 5/22 Final electrical design drawing due out next week. 5/29 By Friday, June 1st. ACTION BY: PEPIN 5/22 E&K to address EIFS Soffit at sidewalk between Fashion Bug and former Movie Gallery. 5/29 EIFS remain as is, adjust sidewalk slope back to 12’-0” +/-. 5/29 Sidewalk at Fashion Bug and General Cleaners will be replaced as per Big Y. ACTION BY: PIONEER VALLEY 5/29 Canopy Soffit width will be 3’-1” at Staples and left of Fashion Bug storefront only. Other areas will be 2’-2” to match Phase I. This work will need to be coordinate with existing utilities plan and new utility work scheduled for Big Y building. 4/3 Need gas company to cut and gas pipe feed Fashion Bug slab below. 4/17 Provide gas service update for next meeting. 5/1 Set up meeting with Baystate to review action plan to abandon gas line below Fashion Bug. 5/8 Gas company will coordinate work with local supervisor and confirm date for cut and cap. 5/15 Bay State Gas new service application received this date.
