I am writing to request that my name be removed from the above referenced builcling permit as the
Contractor of Record and Authorized Agent.
I have not worked on this job for some time and am no longer providing management services on this
project.
I believe that Jim VanNatta will be taking over as the Contractor of Record. I have provided Jim with
copies of all of the original pemlit application documents for his records.
