May 31, 2016 Banas & Chen Real Estate Attn: Michael Banas 63 Main Street Easthampton, MA 01027 Re: Asbestos Abatement Project at 32 Main Street, Northampton, MA Dear Mr. Banas: This letter is to confirm the completion of the asbestos abatement project at 32 Main Street, Northampton, MA per the following scope of services: A.) Removal and disposal of 2x4 wall partition and strapping from the 2nd, 3rd and 4th floors. B.) 1st Floor – Demolition and abatement of the following items. The abatement will be done under negative pressure containment. Post abatement visual inspection and air testing will be provided by an independent certified hygiene firm (Green Environmental).  Ceiling and wall demolition down to the existing brick or joists (similar to above floors).  Removal of VAT, carpet & plywood subfloor.  Bulbs and ballasts will be properly packaged and disposed of. This project was conducted on April 7, 8 & 11, 2016, and was performed in compliance with all applicable regulations. The asbestos waste generated from this project is currently being stored in our locked, 100-cubic yard temporary storage container located on our premises at 483 Shaker Road in East Longmeadow, Massachusetts. Once this temporary storage container is full, the waste will be picked up by a licensed hauler and transported to the final disposal site. Abide, Inc. will send you the original Asbestos Disposal and Documentation Form, the original air clearance report from the independent certified hygiene firm, and a copy of the Massachusetts DEP notification in the documentation report.
