In accordance with the 8th Edition of the Massachusetts State In accordance Building Code, 780
CMR §107.6, I hereby state that, to the best of my knowledge, the construction of the structural work
for the first floor of this project was satisfactorily completed per the design intent as shown on the
drawings F1, S1, S2, S5 & S6 that I had prepared for this project.
I made periodic visits to the site and was in contact with the Architect and the General Contractor
during construction to review the progress of the work. Shop drawings were submitted for review of
structural steel.
Nothing in this document relieves the General Contractor of its responsibility regarding the
provisions of 780 CMR 107.
