Sylvester Rd Keiter ZPACity of Northampton Massachusetts DEPARTMENT OF BUILDING INSPECTIONS 212 Main Street ● Municipal Building Northampton, MA 01060 Scott Keiter 51A Hatfield St Northampton, MA 01060 March 21, 2013 Dear Mr. Keiter, I have reviewed your zoning permit application for a driveway and a single family house on Sylvester Road in Florence. I have approved this application with conditions. The driveway is approved as shown on the plan submitted as long as it is constructed in accordance with the Fire Department requirements. Additionally, you must obtain a permit from the Conservation Commission if required, and a storm water permit from the Department of Public Works if the project, including the house site, disturbs more than an acre. You may construct a single family house on this property, served by the proposed driveway, as long as the house location meets all the setback requirements for the Rural Residential District. Feel free to contact me if you have any questions. Respectfully, Louis Hasbrouck Building Commissioner City of Northampton (413) 587-1240 lhasbrouck@city.northampton.ma.us File # MP-2013-0080 APPLICANT/CONTACT PERSON SCOTT KEITER ADDRESSIPHONE 51A HATFIELD ST (413) 320-9035 PROPERTY LOCA TION SYLVESTER RD MAP 21 PARCEL 004 001 ZONE THIS SECTION FOR OFFICIAL USE ONLY: PERMIT APPLICATION CHECKLIST ENCLOSED REQUIRED DATE ZONING FORM FILLED OUT Fee Paid Building Permit Filled out Fee Paid Typeof Construction: ZP A -DRIVEWAY New Construction Non Structural interior renovations Addition to Existing Accessorv Structure Building Plans Included: Owner/ Statement or License 3 sets of Plans / Plot Plan APff4JJ6j) AS LOCAlCD I 11 fPM(J6{J pdf<-5fH J THE FOLLOWING ACTION HAS BEEN TAKEN ON THIS APPLICATION BASED ON INFOpMATION P~)!N~~D: .. }). c-rrl • 'rt£ (j p~P- --L.LApproved AddItional pemntsrequrred (see below) . ~Vb~ !!J,J,;" '~;~ . f)LJ!16fJT..-S ::::: fi(2£ b tJ~fli RA IV\£/Vr I~L,( Ir<-u f v I PLANNING BOARD PERMIT REQUIRED UNDER: § _______ _ Intermediate Project : ___ Site Plan AND/OR" ___ Special Permit with Site Plan Major Project: Site Plan AND/OR Special Permit with Site Plan ZONING BOARD PERMIT REQUIRED UNDER: § _________ _ Finding. ____ _ Special Permit ____ _ Variance* ____ _ ___ .Received & Recorded at Registry of Deeds Proof Enclosed ____ _ __ Other Permits Required: ___ Curb Cut from DPW ___ Water Availability ___ Sewer Availability lin 0([( (J tJ fr0 feeMIT:5 Rt 47 c,{ lt2W ___ Septic Approval Board of Health ____ Well Water Potability Board of Health r/' Permit from Conservation Commission Permit from CB Architecture Committee --- Permit from Elm Street Commission ---" __ V __ Permit DPW Storm Water Management Signature of Building Official Note: Issuance of a Zoning permit does not relieve a applicant's burden to comply with all zoning requirements and obtain all required permits from Board of Health, Conservation Commission, Department of public works and other applicable permit granting authorities. * Variances are granted only to those applicants who meet the strict standards ofMGL 40A. All other 12% slopes have been approved. • Pavement is not required. • The house may require an independent fire suppression system due to sections of 12% slope. In a conversation with me, Mr. Nichols stated that we can sit down and review the plans together to see if we can compromise somewhere on this point Based on our many discussions with yourself, Carolyn Misch, Dwayne Nichols, and Sarah Lavalley, Jill and I have decided to purchase the land. We thank you very much for all of your time in reviewing this matter. Scott Keiter
