Dear Tony:
Thank you for your quick response on January 10 concerning the
requirement that we apply for an inspection of our mental health
clinic site at 131 King Street . I have enclosed the application
(with our name change) and the inspection fee .
For the sake of clarity, let me trace out in writing who we are.
Northampton Area Mental Health Services merged with Valley Programs
about 18 months ago. On January 1, 1995, Franklin/Hampshire
Community Mental Health Center (F/HCMHC) and Valley Programs merged
to become ServiceNet, Inc . As you are aware, Valley Programs, in
addition to the King Street clinic, operated some residential sites
in Northampton. F/HCMHC operated a mental health clinic at 50
Pleasant Street . I don ' t think any other former F/HCMHC sites fall
under your jurisdiction .
I have been asked by our CEO, Sue Stubbs, to serve as the liaison
to your department . I understand that Jim Frutkin (of ServiceNet )
has already given my name to Frank Sienkiewicz . Our goal in
providing you with a single contact here is to improve
communication and respond quickly and accurately to any requests or
requirements of your department . Please feel free to call me any
time with any questions or concerns .

January 3 , Frank accompanied Joe McEvoy of the Department of

uld like the certificate of inspection to be issued to ServiceNet
