On March 21'2018, 1 inspected 20 Florence Street, Leeds MA(Leeds Elementary School) in
accordance with the Massachusetts State Building Code, 780 CMR chapter 1, §104.4. Below is a
summary of my inspection notes and recommendations.

Exit signs secondary illumination not functioning:
• Pre-school exit by room 004
• Gymnasium
• Hall by room 204
Emergency lighting secondary illumination:
• Play ground stairs and second grade wing — battery pack is out.
• Emergency lighting and hom strobe obstructed by faculty storage shelves.
• Maintain egress corridor in library
Please complete work within 30 days and call to schedule for a re-inspection of the property.
