If registered land,the document number which identifies this transaction is _ Signature Applicant 4/1/94 310 CMR- 430 430 EXHIBIT A DEP NO. 246-389 Ongoing conditions that shall not expire with the issuance of this Certificate of Compliance are as follows: -- Maintenance,to include mowing and removal of woody vegetation,shall be allowed beyond the life of this Order of Conditions. Specifically, the following maintenance tasks will be performed by the City on an annual basis: --- Mowing of the upstream slope, downstream slope,toe and emergency spillway areas of the dam. Mowing must be done not less than once per year and not more than four times per year and will extend to the pond level on the upstream slope and at least twenty(20)feet downstream of the toe of the dam and around the stilling pool. Mowing within the emergency spillway located on the left end of the dam, facing downstream,will extend from the pond level to the limit of the cleared area downstream. ---- Removal of debris around the dam's normal inlet structure and outlet structure and removal of sediment from the stilling pool located at the discharge of the outlet pipe. ---- Operation of the pond drain gate valve at least annually to insure the valve is in good operating condition. ---- Repair of the access road from the Moose Lodge to the darn, as needed.
