Dear Sir or Madam:
On April 11, 2006, anAnnual Liquor License Inspection was conducted at
your establishment. Attached is a list of issues that were identified.
These issues must be resolved within the stated time period:
1. Ansul system must be tied into a hardwired Fire Alarm system that
includes smoke detectors. 60 days
2. Ansul system to be cleaned of grease buildup. 30 days
3. Rear of building and exterior must be cleaned. 30 days
4. Electrical issues must be resolved, including inappropriate use of
extension cords, open boxes and Improper wirirg. 60 days
5. A knox box and strobe must be added. 60 days
6. Need front, basement and kitchen fire extinguishers with signage
above.
Please call when these items have been completed and you are ready for
a re-inspection.
