The State Building Code,780CMR, section 103.1 requires that you maintain the existing building and systems within that building in a safe, operable and sanitary condition. The violations that must be corrected immediately which violate chapter nine in 780CMR are as follows: a. Alarm system that is connected to central monitoring station was not working. b. Pull station did not activate alarm. c. Smoke detectors missing in rooms#'s 4,8,9,12,15,17,19,21 and 23 The above violations muSt be corrected within twenty-four(24) hrs. The next list of violations must be corrected within 30days: a. Front porch 1st and 2nd'and 3`d floor railings broken and missing spindles. b. Skylight broken on top floor—north hall, which in the event of fire would allow fire and smoke to spread to attic area. The shaft consists of a thirty by fourteen inch, sixteen gauge black iron duct, wrapped with inch and a half "Firemaster" duct wrap (specifications are included in the attachment). The wrapped duct will then be enclosed in a USG-750302 two hour fire rated cavity shaft wall (see attached drawings for entire makeup of the two hour wall). Barring any negative recommendations by EDA that slightly skewed beam will be retained. If we lengthen the lintel span where the extended lintel is proposed to this pier, there is additional depth available due to the raising of the second floor. To resolve these questions EDA, at their earliest convenience, will be asked to visit the site and meet with Tony and Dan. 2) In place structural testing of the existing porch brackets was discussed. The General Contractor will undertake this work by suspending appropriate weight from one of the existing second floor porch brackets. Preliminary plans catl for chain falls placed at the existing wall, mid point and end point of the bracket supporting a loading platform just above the sidewalk height; initial loading will be about 7500 lbs., with an additional 1000 lbs. added to validate bearing capacity for porch loading with some slight cushion. It will be necessary to install the vertical channels shown at the interior of the wall and any internal lateral bracing of that channel EDA may require. In conversation with the Architect, Andy proposed substituting paired 4" channels for the tube steel specified to allow for easier bolting to the top of the bracket — also an additional channel pair will need to be added to reduce the span of the decking. EDA will more specifically describe testing technique,duration and required observation. 3) Foundation and rebar drawings have been processed, approved and distributed; anchor bolt layouts are under review. Arrow will need to expedite partial steel framing drawings to allow for installation of the channels and porch bracket testing mentioned above. 4) Rudi distributed elevations and details of the back bar at the first floor. These wilt require revision to reflect the shallow window heights at this wall. The front and back bar below counter level requires further definition. The Owner will supply cuts for proposed beer coolers and bar sinks, indicating their approximate location. General Contractor will layout the loose bar cabinets and tops so that alterations for sinks and equipment (as well as cabinet modifications)can be notated. 5) Handicapped Parking Space .. Preliminary site plan sketches were distributed illustrating the implications of providing required 2% slope at the handicapped stall. Even if the adjacent catch basin is raised these result in a nearly 14% grade in the lot. The Architect now proposes to locate these spaces further toward the rear of the property. Access from this stall will run along the adjacent parking lot perimeter, offsetting at the transformer to allow passage to the rear door. This alteration should allow the catch basin to remain at its existing elevation. This sketch will be revised to delete penetration details for submission al the September le hearing. 8) The use of gyperete in place of specified plywood floor sheathing was discussed for floors two, three, and four. The General Contractor will have a prospective supplier/installer provide the Architect with specific information on parameters for installation, curing, protection/warranty against wood rot contribution, and sound attenuation properties. It is expected that this treatment would extend to the exterior of the new stairwell. A floor flashing product would be used to feather the additional elevation required by the gyperete installation to the top and bottom of stair sections, so that riser variation does not exceed code allowance. 9) The Owner reports That the Fire Inspector verbally approved the fire report submission; Danny will forward this submission to the Building Inspector as requested by the Fire Chief. 10)Tony Bilotta notes that new LPI floor joists directed on structural plans to be sistered to existing floor joists will extend below them. This will apparently require substitution of wood framing for metal stud at the exterior wall at the upper floors. EDA to review and advise. 11)It was noted that the main entry door, shown as a solid unit with glass side light, will be installed at the existing exterior stairs. To protect incoming traffic from exiting patrons it may be wise to substitute a wood/glass unit at this location. Carlson & Schmitt will propose a new door along with suggested hardware. The building department advised me that there was a demolition permit pulled for that location. I walked through the premises and found all detectors and pull stations to be in place and appeared to be operational. I then ran a test of the system and all audible alarms sounded throughout the building and a signal was transmitted to their alarm company. I advised the working crew that a permit needed to be pulled if work was to be done on the fire alarm system and that the alarm must remain operational for the residence. I spoke to the building owner personally and advised him fully of the situation. The alarm was operational, heat detectors should be attached to the ceiling and that a permit needs to be pulled prior to any work on the fire alarm system is done. I suggested he contact you with any further questions.
