Subject: Certificate of Inspection Violations
Dear Ms. Hyde,
I made a visit to 10 Pearl Street on Friday January 2, 1997 and found several serious life
safety violations:
1. No posted or valid Certificate of Inspection.
2. Emergency lights not operating on upper and lower levels
3. Exit lights not operating on upper and lower levels.
4. Stair case areas being used to store equipment impeding means of egress.
5. Use of extension cords in lower bar service area.
On March 17, 1997 an inspection was done and the inspector noted violations for
emergency lights not operation and exits lights not operational. The inspector was to be
called back when corrections were made so that a Certificate of Inspection could be
issued., as of today's date you did not call back for re-inspection .You do not have a
Valid Certificate of Inspection. You have 48 hrs. in which to correct the violations listed
above and call for a re-inspection. If the corrections are not made the club will face being
shut down until the safety corrections are completed
I spoke with Mr. Kaplan your attorney this morning , Mr. Kaplan is aware of the
violations and states that the corrections will be made and re-inspected before January 9,
1997.
