Re-inspection on January 7, 1998'
Dear Ms. Hyde,
A re-inspection was done at Pearl Street Club on January 7, 1998 at approx. 1:30 PM The
following are violations that are still present:
1. Main entry foyer emergency egress light not operational, missing battery.
2. No exit signs oremergency lights in pool table area lower level.
3. Rear Service hallway exit light not operational.
4. Second level Ballroom 2 emergency lights not operational.
5. All bar(south) fire extinguishers;and all stage fire extinguishers out of date.
6. East bar no fire extinguisher.
7. Basement circuit breaker box- meds blanks.
8. Pool room Bar receptacles for pmol table lights pulled out of electrical box, no
covers.
9. Pool room bar pin ball machine plugged into ceiling outlet by extension cord
draped over ceiling mounted radiator.
10. Christmas lights missing bulbs (no longer in use) should be removed.
11. Service entrance exit light not working.
12. Service entrance open junction lox.
13. Service area, THHN wire run on service and through wall to electrical room.
14. First floor tap bar area, extension cord wire used for fixed wiring method on
lights and outlets.
15. Electrical Room Beverage boxeg, stacked against electrical switch gear-need
3 6"clearence,
16. Electrical room panel covers missing.
17. Electrical room Sound equipment wired into circuit breaker box with illegal
ground splice.
18. Light fixture pulled from ceiling.
19. First floor dance area stage lighting interconnect wire strung on ceiling.

20. VY floor dance area strobe light''plugged into extension cord strapped to a
steam pipe.
21. 1". floor dance area Christmas lights on exterior windows bulbs missing- lights
should be removed.
22. Back stairway 2 receptacles & 1 switch missing from box
23. Second floor dance hall no emergency lights
24. Second floor dance area stage receptacle letter E broken.
25. Second floor dance hall Stage panel ungrounded.
26. Second floor dance hall sub panel improper wiring method.
27. Main stairway to second floor switched receptacle on extension cord
unswitched through stairway and ungrounded.
Please contact this office as soon as repairs,are made so that a re-inspection can take
place.
