Standard Permit Conditions: 1. The Northampton Department of Public Works (DPW) or its authorized representative shall be provided access to the property to conduct inspections before, during, and after construction activities as necessary. The applicant shall arrange with the DPW to schedule the following inspections: Initial Inspection, Erosion Control Inspection, Bury Inspection, and Final Inspection. The DPW may also conduct random inspections to ensure effective control of erosion and sedimentation during_all_phase_s___o_feonstruction. 3. At the completion of the work, including construction of stormwater management facilities and landscaping, the applicant shall submit a statement to the DPW from a professional engineer of record in the Commonwealth of Massachusetts certifying that the materials and construction of the stormwater management system was constructed in accordance with the approved plans and will function as designed. All changes to the approved stormwater facilities shall be documented by a professional engineer of record in the Commonwealth of Massachusetts and approved by the DPW prior to submission of the certification statement described above. 4. Annual Reports that are stamped by a Registered Professional Engineer documenting the actions completed as required by the Operation, Maintenance and Inspection Agreement shall be submitted to the Northampton Department of Public Works by October 1" of each year. 5. If construction of the proposed stormwater system has not started within five years of the issue date of this permit, then the permit will expire and a new permit will be required. Construction is defined as clearing, grading, excavation, filling, and installation of drainage structures on the lots specified in this permit. Specific Permit Conditions: 1. Prior to issuance of a Building Permit, the applicant shall record an executed Stormwater Operation, Maintenance, and Inspection Agreement that is binding on all subsequent owners of land served by the private stormwater management facility by recording it in the land records of the Registry of Deeds. This agreement shall be approved by the Northampton DPW and signed by the owner and the Mayor of Northampton. Section 22- E:AStormwater Perm its\Stormwater Permits\Permits new applications\Village Hill(Phase 2).doc Page 2 of 4 85 of the Northampton Stormwater Management Ordinance provides additional guidance for the requirements of this agreement. 2. This project will be required to submit one or more Notice of Intent (NOI) to the U.S. EPA for coverage under the NPDES Stormwater Construction General Permit. A copy of the final NOI and Stormwater Pollution Prevention Plan (SWPPP) shall be submitted to the Northampton DPW prior to the start of construction and prior to issuance of a building permit. 3. The plans and specifications approved by this Permit include roadways and conceptual impervious surface areas for site development. During the Site Plan approval, 40R site approval, or any other permitting for each lot, the applicant must submit documentation to the Northampton DPW of the proposed drainage system for the site, the impervious surface footprint of the site, and a revised summary of impervious surface areas built, proposed, and available for future construction for the sub-watershed drainage area where the proposed lot development is taking place. At certain stages (to be agreed on by the applicant and the DPW) of the build-out of the lots, the applicant shall be required to submit revised drainage calculations that reflect the actual construction that has been completed and confirms the remaining capacity of the Detention Basins in each Sub- Watershed area. If the overall size of the impervious surface footprint proposed for each — ---_---_--_ _sub-watershed-as defined-in-the-drainage calculations-is-greater-thanthe-area-specified-in--- the n-the approved drainage calculations or if the drainage system is changed in a substantive way, the applicant shall be required to submit revised plans and drainage calculations and may be required to submit a new application for a Northampton Stormwater Management Permit for the proposed site plan. 4. The Existing Conditions Plan Sheet 1.2 shows a Sediment Forebay in Detention Basin 1. The DPW has commented in the review of As-Built plans for this Detention Basin to the applicant that this forebay does not appear to be constructed. Due to the level of the permanent pond in the Detention Basin, it is apparent that the forebay would not function according to the original design. Prior to issuance of a building permit, the applicant shall design and construct a forebay that meets the approval of the DPW. The forebay will function to allow sediment to settle out and provide an efficient means to conduct routine maintenance to remove sediment from the basin without having to drain the entire basin. 5. Following the excavation of existing pavement for the construction of Musante Drive, the contractor must take steps to reduce compaction of the soils in the area of the proposed Water Quality Swales. The proposed Water Quality Swales must be constructed and stabilized prior to receiving drainage flows from the roadway and shall be fenced or otherwise protected during construction of the roadway to prevent heavy equipment from compacting the underlying soil and prevent sediment washing into the swales. 6. All stormwater systems shall be constructed as proposed and stabilized prior to receiving drainage flows. 7. Within 60 days of the issuance of this permit, the applicant shall submit a revised Musante Drive Plan and Profile, Sheet 5.3 specifying a clean out for each perforated pipe proposed in the Water Quality Swales at the opposite end to the proposed overflow inlets. E:AStormwater Permits\Stormwater Permits\Permits_new applications\Village Hill(Phase 2).doc Page 3of4 8. The plans show a proposed Drainage Easement on Lot 13-A for proposed Detention Basin 3. Additional drainage easements may be necessary for the proposed Water Quality Swales on 38A-087. The Water Quality Swales shall be required to be maintained by the property owners and must be included in the Stormwater Operation, Maintenance, and Inspection Agreement as described above. The DPW must review and approve all proposed Drainage Easements prior to recording at the land records of the Registry of Deeds. 9. The proposed Stormwater Outfall shown on Drainage Outfall Plan & Profile, Sheet 5.6 specifies 1:1 slopes on the sides of the outfall structure as well as 2:1 slopes above the proposed headwall. The applicant shall modify the design as much as is feasible to reduce the pitch of these slopes per approval of the Northampton Conservation Commission and the DPW. 10. If the design of the proposed Stormwater Outfall changes during the Northampton Conservation Commission approval of Notice of Intent, the applicant shall submit the revised design to the DPW for approval as an amendment to this permit. 11. Within 60 days of the issuance of this permit, the applicant shall submit documentation that all structures in the proposed stormwater collection system have been sized to adequately collect and convey runoff from the 25-year storm event. n 1 Signature of City Engineer 1 Dates / t cc.
