Approval of the Stormwater Management Permit Application. X Approval of the Stormwater Management Permit Application subject to any conditions, modifications or restrictions required by the Department of Public Works (see below) Disapproval of the Stormwater Management Permit Application based upon a determination that the proposed plan, as submitted, does not meet the purposes set forth in the Northampton Stormwater Management. Standard Permit Conditions: 1. All clearing, grading, drainage, construction, and development shall be conducted in strict accordance with the following plansand information submitted with the application and as amended by any conditions attached to this permit. a. The Northampton Department of Public Works (DPW) or its authorized representative shall be provided access to the property to conduct inspections before, during, and after construction activities as necessary. The applicant shall arrange with the Northampton DPW to schedule the following inspections: Initial Inspection, Erosion Control Inspection, Bury Inspection, and Final Inspection. The Northampton DPW may also conduct random inspections to ensure effective control of erosion and sedimentation during all phases of construction. 3. Annual Reports documenting the actions completed as required by the Operation, Maintenance and Inspection Agreement shall be submitted to the Northampton Department of Public Works by November 1St of each year. 4. If construction of the proposed stormwater system has not started within five years of the issue date of this permit, then the permit will expire and a new permit will be required. Construction is defined as clearing, graditig, excavation, filling, and installation of drainage structures on the lots specified 10 this permit. Specific Permit Conditions: 1. The applicant must submit to the Northampton DPW, within 30 days of the issuance of this permit, a complete package of revised and updated plans, stormwater management calculations, and other supporting documentation that includes all changes made since the applicant's submission of: 1) The Village,at Hospital Hill, South Campus Site Development Plans, Sheets 1-34, dated 12/09/2004 by Beals and Thomas, Inc. and 2) the Planning Board Application for Major Project Site Plan Approval for The Village at Hospital Hill South Campus—Tier I Review, dated 12/10/2004, by Beals and Thomas, Inc. The Northampton DPW shall confirm to the applicant in writing that all revisions have been made to the satisfaction of the Northampton DPW. Other information to be included in the revised documents noted above: a. The revised Invert Schedule shall include complete and accurate information. The applicant shall provide missing invert information for DMH-48, DMH-54, DMH- E:AStormwater Permits\Stormwater Pennits\Pennits_new applicaitions\Prince Street-South Campus Hospital Hill.doc Page 2 of 5 - - - 55, DMH 56, FE O1, CB 55, CB $6, CB-57. This 1>at may not be complete, and the applicant shall make every effort to correct the accuracy of the information submitted. b. The revised stormwater management calculations shall include a more detailed analysis of DP-4 which receives drainage flow discharged from Detention Basin 5 and the proposed underground Detention-05, through Wetland Area 3, and flows either to the municipal drain line on Grove Street (vitreous clay,built 1932) or crosses Grove Street to a wetland area and drainage ditch next to the railroad tracks. The Northampton DPW is investigating the route of the pipes offsite and will forward information to the applicant. Since proposed peak flows to DP-4 are only a slight reduction from existing peak flows to EDP-4 (and an increase to peak flows approved by the Definitive Subdivision Plan) and existing flows can be problematic and have resulted in flooding and icing onto Grove Street, the applicant shall analyze the capacity of the existing inlet and municipal drain line to safely convey flows to the municipal system or offsite via another route. The applicant must also include calculation assumptions for the flow through Wetland Area 3 that appears to include Addlink 100, OCS-6, and Route 130 in the drainage routing diagram. 2. During the Tier II Site Plan approval for each lot, the applicant must submit documentation to the Northampton DPW;of the drainage system and the impervious surface footprint of the site. If the size of the impervious surface footprint proposed for the Tier 11 Site Plan Approval is greater than the area specified for the Tier I Site Plan Approval or if the drainage system is changed in a substantive way, the applicant shall be required to submit a new application for a Northampton Stormwater Management Permit for the proposed site plan. 3. The drainage from the proposed 45,000 sq. ft. building on Lot 12A has not been discussed or included in the drainage calculations. The applicant will be required to submit a separate application for a Northampton Stormwater Management Permit for this building. 4. The Letter to the Northampton Department of Public Works from Beals and Thomas, Inc. dated 01/21/2004 regarding DPW comments on the Phase I Definitive Subdivision Plan states: "Demolition plans will be submitted during the site plan review process." During recent meetings the applicant has stated that this information will not be submitted, therefore the applicant shall be required to notify the Northampton DPW that appropriate measures have been taken to remove and/or plug all existing drainage systems as they are encountered during site development. 5. The applicant shall submit a revision of the documents: "Reconfigured Infiltration/Detention Sketch SKC-001 and SKC-002," prepared by Beals and Thomas, Inc. dated 01/05/2005, that provides two Clean-Out/ Inspection Ports for each subsurface infiltration and detention basin—one located adjacent to the inlet and one located adjacent to the outlet of each basin. 6. The applicant shall submit a signed and completed MA DEP Form 11 –Soil Suitability Assessment for On-Site Sewage Disposal for each test pit previously conducted to ensure that a Certified Soil Evaluator has determined the information submitted. The applicant E:AStonnwater Pennits\Stormwater Pennits\Pennits_new appl i cation s\Prince Street-South Campus Hospital Hill.doc Page 3 of 5 R shall conduct one additional test pit in the location of the proposed subsurface Detention Basin 05 to determine ground water elevation and estimated seasonal high groundwater elevation. A signed and completed DEP Form 11 shall be submitted to the Northampton DPW to document this test pit. 7. Before the development site is graded, the areas where the proposed subsurface infiltration basins will be constructed shall be fenced to prevent heavy equipment from compacting the underlying soil. S. The applicant must record an executed Stormwater Operation, Maintenance, and Inspection Agreement that is binding on all subsequent owners of land served by the private stormwater management facility by recording it in the land records of the Registry of Deeds. This agreement shall be signed by the applicant and approved by the Northampton DPW. Section 22-85 of the Northampton Stormwater Management Ordinance provides additional guidance for the requirements of this agreement. The Northampton DPW will supply a template to the applicant for this agreement. a. The Operation, Maintenance, and Inspection Agreement shall include requirements that the applicant provide documentation that the three subsurface infiltration basins and one subsurface detention basin continue to function as designed. Regular inspection and maintenance of the infiltration systems shall be included in the Stormwater Operation, Maintenance, and Inspection Agreement. In order to inspect the infiltration systems, two observation wells shall be installed in each basin to determine how quickly the infiltration basins drain after a storm. The observation wells should be perforated PVC pipe, 4-6"diameter, extending to the bottom of the bed where they are connected to a foot plate. The observation wells should be capped and locked to prevent vandalism or tampering. Ponded water inside the infiltration basin (as visible from the observation well) after 48 hours or several days can indicate that the bottom of the basin is clogged. The owner is required to repair, replace, or reconstruct the infiltration system if it fails to operate as designed. A system fails to operate as designed when water is standing 48 hours or longer following a storm event greater than 1 inch over 24 hours. 9. The Erosion & Sediment Control Plan submitted by the applicant provides a good initial framework for the entire site construction!. Additional details and adjustments to this plan shall be provided to the Northampton DPW as individual sites are developed. The applicant shall provide the following: a. Each site development will be required to submit one or more Notice of Intent (NOI) to the U.S. EPA for coverage under the NPDES Stormwater Construction General Permit. Each site will be required to prepare and implement a Stormwater Pollution Prevention Plan (SWPPP). Copies of both the NOI and SWPPP for each site shall be submitted to the Northampton DPW prior to issuance of a building permit. The SWPPP for each site shall be reviewed by the Northampton DPW and may need to be amended to the satisfaction of the Northampton DPW. b. The applicant shall remove the proposed use of haybale check dams around existing and proposed drain inlets'',in the "Erosion Control and Sedimentation Notes" on Sheet 2-Erosion and Sedimentation Details and Notes and replace with E:AStonnwater Permits\Stonnwater Pennits\Pennits_new applications\Prince Street-South Campus Hospital Hill.doc Page 4 of 5 the proposed use of siltsacks/filter bags, gravel check dams, or other appropriate measures to better protect drain inlets.
