Consulting Engineers Date of Submittal: December 20, 2004 SECTION 1 —BUILDING DESCRIPTION PROJECT SCOPE: This project consists of renovation to the former South Building to create new multi-family living units. This project will include a new wet pipe sprinkler system to serve the entire building and a new addressable fire alarm system to allow monitoring and supervision of the new sprinkler equipment and to meet current codes and regulations. The South Building is located at Village Road. The Building Use Group is Residential R-2 with Construction Type 3B, Combustible/Unprotected as defined by the Massachusetts State Building Code. The building is three stories with a full basement and finished attic. This project will consist of a complete renovation of the entire building to create new apartment units. The entire building is approximately 14,000 s.f. The area affected by this project will be 14,000 s.f. Fire Department personnel and rescue vehicles can access the site on all four sides of the building. SECTION 2 -APPLICABLE LAWS, REGULATIONS & STANDARDS Systems were designed in compliance with the following: 1. 780 CMR 9, "Fire Protection Systems". 2. NFPA 72—2002, "National Fire Alarm Code". 3. NFPA 13-2002, "Installation of Sprinkler Systems". 4. 521 CMR 40 "Architectural Access Board". L&S 1171 Page 2 SECTION 3 - DESIGN RESPONSIBILITY The design of the new fire protection sprinkler system and fire detection and alarm systems were prepared by: Lindgren & Sharples, P.C. 435 Cottage Street Springfield, MA 01104-4005 (413) 732-4336 SECTION 4 - FIRE PROTECTION 1.0 Basis of Design The building will be served by a wet pipe fire protection system consisting of sprinklers located in all occupied spaces and the attic. These sprinklers are connected to a new sprinkler riser and wet pipe alarm valve. All living spaces will be designed for light hazard and all storage and mechanical spaces, including laundry area designed for ordinary hazard. 2.0 Installation A. The fire sprinkler system is designed in accordance with NFPA 13, 2002. B. All spaces are provided with quick response sprinklers. 3.0 Equipment A. Pipe is to be schedule 40 steel pipes for 1" through 3", pipes 3 '/z" and larger may be scheduled 10 steel. B. A main flow switch is provided in the main sprinkler line and service will be fitted with an approved backflow preventer. Flow switches will also be installed at each floor for zoning and isolation purposes. 4.0 Design Criteria A. Each living unit is designed for light hazard occupancy. The density is .10/1500 square feet with a100 gpm hose allowance. B. The storage and mechanical areas, including the laundry area, throughout the building that are ordinary hazard, the system will be designed to ordinary hazard group, density with 250 gpm added for hose stream. L&S 1171 Page 3 5.0 Special Design Criteria A. All sprinklers in mechanical equipment spaces shall be fitted with wire cages. B. Sprinklers are not installed in closets not over 12 square feet as outlined in NFPA 101 paragraph 18.3.5.1. 6.0 Test Criteria A. All sprinklers and standpipe systems shall be tested in accordance with NFPA 13 and NFPA 25. Complete system flow test will be conducted and test and flow certificates issued. 1. (WET PIPE SYSTEM) An alarm test connection not less than 1 in. in diameter, terminating in a smooth bore corrosion-resistant orifice, giving a flow equivalent to one sprinkler of a type having the smallest orifice installed on the particular system, shall be provided to test each water flow alarm device for each system. 2. (WET PIPE SYSTEM) The test connection valve shall be readily accessible. 3. (WET PIPE SYSTEM) The discharge shall be to the outside, to drain connection capable of accepting full flow under system pressure, or to another location where water damage will not result. B. Hydrostatic Test 1. Hydrostatically test the entire interior piping system at not less than 200 psi (13.6 bars) for 2 hours and provide a test certificate for said test. C. Flow Test 1. A system flow test shall be performed and the operation of all flow switches, tamper switches, and alarm devices verified. A test certificate shall be issued verifying said test. SECTION 5 - FIRE ALARM SYSTEMS 1.0 Basis of Design A. System connected smoke detectors will be installed in corridors, stairwells, equipment rooms and storage rooms. Activation of any system smoke detector will initiate the fire alarm system sequence of events. L&S 1171 Page 4 A fire protective signaling system will be installed in accordance with 780 CMR and NFPA 72. B. Bedrooms will be equipped with multiple station smoke detectors. Handicapped bedrooms will be equipped with multiple station smoke detectors and with strobe light connected to the FA system. C. Fire protection signaling and detection devices in this mixed use building will include the following features: 1. Manual pull station within 5 feet of each exit door, all pull stations shall be dual action. 2. Audio and visual notification devices as shown on plans. It is the intent of this design that audible notification devices achieve sound pressure levels of 15 dba above ambient sound levels. The minimum sound level shall be as follows: a. Residential Occupancy—70 dba 3. Visual alarm devices shall be provided in common stairway areas and as specified in accordance with the Massachusetts State Building Code. System visual notification device shall be installed in handicapped bedrooms. 4. The fire alarm system shall be addressable Edwards System Technology EST-2 FACP, or equal, with a remote annunciator installed in unit of the adjacent building. 5. Emergency power supply for the fire alarm panel shall be supplied by battery packs located at the FACP. D. Call for aid system devices installed in handicapped bathrooms. E. Elevator recall will be interlocked with new panel. F. This project does not include any special design considerations or anticipated deviations from the code requirements. 2.0. Sequence of Operation: A. This new addressable fire alarm system is designed to detect fires normally associated with this type of facility. The Activation of any manual pull station, flow switch heat detector or system smoke detector will immediately and L&S 1171 Page 5 simultaneously activate all system audio/visual units and send a signal via the EST-2 Fire Alarm Control Panel and notify the Northampton Fire Department. The system will be constantly monitored as part of the control panel's function. Removal of any fire alarm device or any system wiring fault such as opens or grounds will initiate a trouble alarm at the Fire Alarm Control Panel and send a trouble signal to the monitoring system. The Fire Alarm Control Panel reset button returns the system to its normal state. Display messages shall provide operator assurance of the sequential steps as they occur. The Fire Alarm Control Panel is equipped with a 24-hour battery back up. The Fire Alarm Control Panel is also equipped with visible and audible signals to indicate alarm and trouble conditions. B. The activation of any apartment bedroom multiple station smoke detector will immediately and simultaneously activate all multiple station smoke detectors in the unit. These devices are local the unit and will not transmit a signal to the Fire Department. C. The activation of a call for aid device will transmit an alarm to the exterior of the living unit. It was also activate local annunciator and a location beacon on the exterior of the buildings. 3.0 Test Criteria: All features, devices, components, and modes of operation shall be tested for acceptance in accordance with the code and NFPA 72. Smoke detectors shall be activated using a magnet per the manufacturer's recommendations and all pull stations shall be activated by opening and upon activation of each device the proper operation of the Fire Alarm System shall be checked for proper operation including annunciation at the Fire Alarm Control Panel and transmission of an alarm signal to the Public Safety Department. Sequence of Operations and Interactions HVAC • Heating will be provided by a gas-fired hot water boiler. Each living unit is heated by wall mounted finned tube radiation with one zone per apartment. All storage and mechanical spaces will be heated by hot water unit heaters. Lighting • All lighting in the mechanical rooms and corridor area will be controlled by local switches. • Emergency lighting will utilize normal light with delays to turn on designated lights and exit sign lighting will be fed directly (un-switched) local panel. Emergency lighting is battery operated. 4. Systems and Equipment Capacities HVAC • The HVAC system consists of six modular gas fired hot water boilers with a modulating boiler control panel. Hot water is distributed to a two pipe reverse return piping system and an inline circulating pump with premium efficiency motors. Page 3 Each apartment bathroom exhaust and kitchen exhaust is served by a locally controlled ceiling fan. • Lighting and power systems have been designed to provide adequate foot candle levels, power distribution, fire alarm, and communication. Lighting • The ambient lighting system for the new Mechanical Room will be accomplished with a combination of direct and indirect fluorescent fixtures. The lighting controls will be located locally and operated with ease. 5. Testing • The following tests and inspections shall be performed by the General Contractor with the Owner and the Owners' representative present. The test and inspections must comply with the criteria stated for the work to be acoepted. HVAC • Inspect new radiators, boilers, and pumps for any physical damage. There shall be no dents or distortion of the metal. • Inspect new radiators, boilers, and pumps for any unusual noise or vibration. There shall be no unusual noise or vibration when the unit is operating. • Inspect new piping and equipment for leaks. • The HVAC system shall be tested to verify that the final TAB report is correct. Readings within 10% of those listed on the final TAB report shall be considered acceptable. If the readings are not. acceptable, then all systems in the space must be balanced and a new final TAB report prepared. After the new final TAB report is issued, all HVAC systems in that space will be tested to verify that the new final report is correct. • Verify that the HVAC system is functioning correctly. The control valves and pumps shall operate when the space sensor calls for heat. Lighting Page 4 • Demonstrate that the time clocks controlling the lighting system automatically turn the lights off and on at the prescribed times. • Inspect that all lamps in all the fixtures are functioning. 6. Operation Manuals and Maintenance Manuals When the Fire Headquarters is completed and ready for occupancy, the General Contractor shall provide the following items before the review and final testing will begin. HVAC • Two complete sets of operating manuals. • Two complete sets of maintenance manuals. The manuals shall cover all new HVAC equipment. Electrical • Two complete sets of operating manuals. Installation of silt fence and haybale erosion control measures. 2. A stone truck pad will be constructed at the project entrance to capture sediment and control tracking of soil onto roads outside the project limits. 3. Topsoil cleared and stockpiled within limits of work and additional silt fence installed around base of pile to control run-off. i 4. Sewer, water and drainage infrastructure will be installed within the confines of the project limits only. (Main storm water conduit and detention system to be 3 installed as part of an adjoining project managed by the MassDevelopment Agency). 5. Electric, gas, telephone, cable utilities (subsurface) will be installed within the confines of the project limits only. 6. Installation of bituminous pavement and berm per plan and spec. 7. Complete rough grading, spread topsoil and establish lawns and plantings. 8. Remove accumulated sediments from all drainage structures. 9. Remove silt fence barrier after lawns have been established and all danger of soil erosion has been eliminated. Estimate of Disturbed Area The project site covers approximately 2.75 acres. Construction activities will result in the disturbance of approximately 2.75 acres. Erosion control barriers will be installed prior to clearing and excavation work. 2. Grading and other soil disturbance will be done so as to minimize erosion during wet seasons. 3. Two storm water retention basins will be constructed as part of a separate contract known as the Village Hill Road project, overseen and managed by the MassDevelopment Agency. All storm water runoff collected within the Village at Hospital Hill renovation project will be piped to a central storm water collection j system and directed into a series of two detention basins located outside the project limits but still on the grounds of the Northampton State Hospital. 4. Sediment control barriers will be inspected regularly and assessed to assure their i proper function. 5. The clearing of natural vegetation will be minimized and remaining natural vegetation will be protected to the greatest degree possible. 6. Staging and soil stockpile areas shall be located at least 100 feet from wetlands. A hay bale/siltation fence barrier will be installed immediately down-gradient of such areas. 7. Designated temporary dewatering basins will be used for dewatering. ' 8. Disturbed areas will be stabilized as soon as possible after construction. 9. Maintenance and cleaning of construction vehicles and equipment will take place in designated staging areas only. Structural Practices The following structural erosion and sediment controls will be utilized during construction in order to maintain local water quality: 1. Hay bales/Filter Fabric Fencing: Staked hay bales with filter fabric fencing will be installed prior to site clearing and between any wetlands and areas of work. • Filter fabric will be set into ground 4" to 6". • Sediment will be removed from behind silt fence when it reaches half the original height of the fence. • Silt fences will be inspected and repaired or replaced as necessary. • The site contractor will be responsible for implementation of this BMP. 5 Village at Hospital Hill—Phase/ Storm Water Pollution Prevention Plan 2. Dewatering Basins: Dewatering basins will be constructed where required prior to excavation activities. These basins will act to settle suspended solids from pumped groundwater. • Basins will be sized according to the amount of groundwater encountered at a particular location. • Basins will utilize a perforated standpipe wrapped in filter fabric for discharge. • Sediment will be removed when it reaches half of the original height of the filter. • The site contractor will be responsible for implementation of this BMP. 3. Catch Basin Filters: Filter consisting of anchored hay bales, embedded 4-6" in the ground, will surround each new catch basin. • Filters will be placed around each catch basin prior to paving or planting. • Sediment will be removed when it reaches half of the original height of the filter. • Filters will be removed only after up-gradient areas have been permanently stabilized. • The site contractor will be responsible for implementation of this BMP. 4. Dust Control: Dust control will be maintained by sprinkler or water truck during construction to minimize sediment transport and maintain air quality at an acceptable level. • The site contractor will be responsible for implementation of this BMP. 5. Roadway Stabilization: Until final- paving takes place, project driveways and parking areas will be stabilized by grading with clean gravel. • The site contractor is the operator responsible for implementation of this BMP. Stabilization Practices The following stabilization practices will be utilized during construction in order to maintain local water quality: 1. Temporary stabilization: temporary deeding, mulching or other suitable stabilization measures will be utilized to protect disturbed areas and stockpiles during prolonged construction periods. 2. Permanent stabilization: areas disturbed by construction will be permanently stabilized by paving with concrete or bituminous concrete, by installation of plant material, or by seeding and mulching with seed mix as described in the project specifications. Seeded areas will be covered with straw mulch to protect surface until seed germination. 6 Village at Hospital Hill—Phase/ Storm Water Pollution Prevention Plan , Timing of Controls As indicated in the Sequence of Major,Activities, the haybale/silt fence barrier and stabilized construction entrance will be constructed prior to clearing or grading of any other portions of the site. No excavation or dewatering activities will take place in an area until appropriate dewatering basins or sediment control structures have been installed. Areas where construction activity temporarily ceases for more than 21 days will be stabilized with temporary seed and mulched within 14 days of the last disturbance. Once construction activity ceases permanently in an area, that area will be stabilized with plant material or pavement as indicated on the plans. After the entire site is stabilized the haybale/silt fence barrier will be removed. MAINTENANCE & INSPECTION PROCEDURES Construction Dates (Record and Maintain) The following events are to be recorded and incorporated into the SWPPP: { Dates when major grading activities occur • Dates when construction activities temporarily or permanently cease on a portion of the site. • Dates when stabilization measures are initiated. The project general contractor(or site contractor if there is no general contractor for project) is the operator responsible for recording the information dates listed above. Form 1 of the SWPPP has been included for recording this information. Form 1 is attached to the back of the SWPPP. Practices to Limit Erosion From Exposed Areas Structural BMPs will be implemented to limit runoff and the discharge of pollutants from exposed areas of the site. Exposed areas will stabilized according to the plans and in compliance with the stabilization practices described above. Post-Construction Storm Water Management A system for storm water management has been designed for this project. The system, designed by a professional engineer, utilizes curb and gutter, catch basins and storm water treatment chambers. Water collected via these means will be directed via buried pipe into a two-stage detention pond system to be built by the associated Village Hill Road project managed by the MassDevelopment Agency. 7 Village at Hospital Hill—Phase,/ Storm Water Pollution Prevention Plan Waste Disposal 1 Waste Materials: The general contractor shall ensure that all construction and demolition debris will be placed into metal dumpsters rented from a licensed solid waste management company. Dumpsters will be emptied as necessary and disposed of in accordance with local, state and federal regulation. No waste materials will be buried on site. The general contractor will also assure that all scrap metal will be separated from the waste stream and deposited into specially marked dumpsters for removal by a licensed scrap metal dealer. All Massachusetts State laws pertaining to recycling of waste paper, cardboard, glass and metal will be observed and containers to facilitate separation and collection will be placed at the project manager's office trailer. Notices stating these procedures will be posted outside the job trailer and all site personnel will be instructed in these procedures and site construction supervisor(s) will ensure that these procedures are followed. i Hazardous Waste: Hazardous waste will be disposed in the manner specified by local, state and federal regulation or by the manufacturer. Site personnel will be instructed in these procedures and site construction supervisor(s) will ensure that these procedures are followed. Sanitary Waste: The general contractor will provide a sufficient number of portable toilets for use by all workers on the project.. The number of toilet units will be based upon the number of workers on the site, the;number of hours worked weekly and the standard recommendations of the sanitary waste contractor. Portable toilets will be maintained in a clean and sanitary manner at all times. Portable toilets will be emptied and cleaned on a weekly basis by a licensed sanitary waste contractor. Off-Site Vehicle Tracking A stabilized stone construction entrance pad will be emplaced to reduce tracking of sediments onto adjacent roadways. Stone large enough to not be picked-up in truck tires, will be installed at the project entrance in order to dislodge sediment from vehicles leaving the site. The paved street adjacent to the construction entrance will be swept regularly to reduce mud, dirt or sediment tracked from the site. Dump trucks hauling material to and from the site will be covered by tarps. Material Inventory for Pollution Prevention Plan It is expected that the following construction materials will be stored at the project site during the construction process: Asphalt Masonry block Cleaning solvents Metal plates Concrete Paints (enamel and latex) Detergents Petroleum-based products Fertilizer Solvents Sand, Stone & Gravel Wood PVC Pipe and Fittings Precast Concrete Pipe & Drainage Structures 8 Village at Hospital Hill—Phase/ Storm Water Pollution Prevention Plan HDPE Pipe Ductile Iron Pipe and Fittings Spill Prevention All employees will be instructed regarding spill prevention practices. Notice of these practices will be posted in the job trailer and the site construction supervisor will hold responsibility for ensuring that all procedures are followed. Material Management Practices The following material management practices will be used to reduce the risk of spills and other accidental exposure of materials and substances to storm water runoff. a Jobsite Housekeeping The following jobsite housekeeping practices will be followed during the construction process to prevent pollution and debris from entering storm water flows: • Materials stored on-site will be maintained in an orderly and workmanlike manner. Tarps or plastic sheeting Will be used to cover materials if required. • Jobsite waste and debris will be cleaned-up and placed into dumpsters daily. • Trash receptacles will be emptied daily to discourage wildlife from entering the work site to eat food scraps left by workers. • Spills will be cleaned-up and reported to the jobsite superintendent immediately. • Smoking will not be permitted in the proximity of any stored construction materials or debris. Hazardous Products The following management practices will be used to reduce risks associated with hazardous materials: • All fuels, oils, solvents, lubricants, faints or other hazardous products will be kept in original containers with labels intact. • Material Safety Data Sheets (MSDS) for each product used on the project will be maintained on the jobsite at all times. • All fuels, oils, solvents, cleaners or other hazardous materials will be stored under a roof or other enclosure with an impervious floor surface designed to contain any and all spills. • Substances will not be mixed with one another unless recommended by the manufacturer. • Manufacturer's recommendations for proper used and disposal will be followed. • The site superintendent will be responsible for ensuring proper use, storage and disposal of all hazardous materials and waste and on-site. • Smoking will not be permitted within fifty-feet of any fuels, oils, solvents, lubricants, paints or other hazardous products. 9 Village at Hospital Hill—Phase I Storm Water Pollution Prevention Plan i } Product Specific Practices Petroleum Products: All on-site vehicles will be monitored for leaks and will receive regular inspections and preventive maintenance to reduce the chance of leakage. Petroleum products will be stored in sealed, clearly labeled containers. No vehicle refueling or maintenance will take place within 100 feet of a wetland or waterway. No petroleum-based products will be stored within 100 feet of a wetland or waterway. All spills, no matter how small, will be reported to the project superintendent and clean-up immediately. Fertilizers: Fertilizer will not be stored on the jobsite. Only that fertilizer planned for use on a given work day will be brought onto the site and any remaining or partial units will be removed from the site at the end of each day. No fertilizers will be used within 100 feet of a wetland or waterway. Fertilizers will only be applied in the minimum amounts recommended by the manufacturer. Once applied, fertilizer will be raked or otherwise worked into the soil to limit the potential for uncontrolled run-off into the storm water system and adjacent waterways. a Solvents, Paints and Other Hazardous Substances: All containers will be sealed when not required for use. Excess material will not be discharged to the storm sewer system but will be properly disposed of according to manufacturers' instruction or local and state regulations. No solvents, paints or other hazardous substances will be stored within 100 feet of a wetland or waterway. Asphalt substances used on site will be applied according to the manufacturer's recommendations. Concrete Trucks: Concrete trucks will not be allower to wash out or discharge surplus concrete or drum wash water into the storm water system or within 100 feet of a wetland or waterway. Spill Control Practices The following practices will be followed for spill prevention and cleanup: • A Spill Clean-up Station shall be established outside the general contractor's field office. The station will be kept unlocked and fully-stocked with appropriate spill containment and clean-up supplies so that workers can react immediately to a spill of hazardous materials. • A log will be maintained to record the date, time, type of spill, and action taken for all spills of hazardous materials. • Spills of toxic or hazardous material will be reported, regardless of size, to the Massachusetts Department of Environmental Protection at 888-304-1133. • Manufacturers' recommended methods-for spill cleanup will be clearly posted and all site personnel will be made aware of the procedures and the location of the information and cleanup supplies. 10 .I Village at Hospital Hill—Phase/ Storm Water Pollution Prevention Plan a i • Materials and equipment necessary for spill clean-up will include, but is not limited to, brooms, dust pans, mops, rags, gloves, goggles, kitty litter, sand, sawdust,plastic bags and sealable containers specifically for this purpose. • All spills will be cleaned up immediately after discovery. • The spill area will be kept well ventilated and personnel will wear appropriate protective clothing to prevent injury from contact with a hazardous substance. • Should a spill occur, the spill prevention plan will be adjusted to include measures to prevent another spill and to cleanup up the spill should another occur. A description of the spill, along with the causes and cleanup measures will be included in the updated spill prevention plan. j • The construction superintendent will be the spill prevention and cleanup coordinator. The superintendent will designate two other personnel to receive spill prevention cleanup and training. The names of responsible spill personnel will be posted at the Spill Clean-up Station at job trailer. Pollutants from Construction Support Sources Not applicable to the project covered by this SWPPP. Non-Storm Water Discharge Management It is expected that the following non-storm water discharges will occur from the site during the construction period: • Water from water line flushing. • Pavement wash waters (where no spills or leaks of toxic or hazardous chemicals have occurred). • Uncontaminated groundwater from dewatering excavations. • Water from washing the exterior of construction vehicles. • Water used to control dust. Non storm water discharges will be directed to stabilized surfaces or the detention basin prior to discharge. Exterior washing and rinsing of vehicles will take place more than 100 feet from wetlands or waterways. Maintenance of Controls Erosion and Sediment Control Inspection and Maintenance Practices The following inspection and maintenance practices will be utilized in this project to maintain sediment and erosion controls: • Silt fence will be inspected for depth of sediment, tears, to verify that fabric is securely attached to the stakes and to verify that stakes are firmly in the ground. 11 Village at Hospital Hill—Phase/ Storm Water Pollution Prevention Plan • Built-up sediment will be removed from silt fence when it has reached one-third the height of the fence. 0 Contractor will stockpile on site or make available all equipment, materials (e.g. filter fabric, hay bales, crushed stone, etc.) and labor necessary to make emergency erosion control improvements within four hours if necessary. • Temporary and permanent seeding will be inspected for bare spots, washouts and healthy growth. • A maintenance inspection report will be made after each inspection. • The site contractor will select one or more individuals who will be responsible for inspections, maintenance and repair activities and for completing inspection and maintenance reports. • The General Contractor will continuously inspect all erosion and sediment controls and report deficiencies to the site work contractor for immediate repair and remediation. i Documentation of Permit Eligibility Related to Endangered Species Not applicable to the project covered by this SWPPP.
