As per our phone conversation of todays date , please be
advised of the following violations on your permit number
89-700 , at 53 Main Street , Florence , Mass . , dated November

I have contacted you before on the violations on this job ,
which still.. exist . Please be advised by receipt of this
letter that the following violations must be corrected.
