signed off on the location and the gravel base grade for the above referenced
driveway. However there is still the existing illegal driveway that can be accessed.
This illegal drive needs to be barricaded in such a way that restricts all exit and egress
over Lot#2. There was an existing drive on Lot 03 that went to the existing barn that
can be used for access to the barn only.

Subject: Violations of permits issued,Map 03 Lot 011 Coles Meadow Road
Mr. Bercume,
I received a letter from the Department of Public Works dated December 13,2000. The
letter from Ned Huntley Senior Engineer states that you have violated the trench permit
issued to you after a December 8, 2000 meeting with your engineer, James Smith,by
performing the work not in accordance with the plans that were presented to DPW at
that December 8, 2000 meeting. The Department of Public works letter also states that
you violated the driveway location permit issued to you for 575 Coles Meadow by d�
moving the location of the driveway that was approved by DPW on 7/12/2000.
The Department of Public Works letter also states that you failed to et a sub-grade
inspection for the driveway at 583 Coles Meadow Road.
Pursuant to these violations no Certificate of Occupancy or further permits will be issued
for those houses or lots until you correct the violations and the Department of Public,
Works issues this department, in writing, an approval that you have completed the work
to their specifications.
The violations could cause revocation of the Building Permits issued to you and you
may face fines and or court action if you fail to correct the violations.

DRIVEWAY PERMIT
GENERAL INSTRUCTIONS AND REGULATIONS
1. A "Driveway Permit" is required in all cases where a new curb cut or an alteration to an
existing curb cut is proposed on a City public way.
2. Driveway permits issued by the Board of Public Works (BPW) shall be attached to and
become part of the 'Building Permit" issued by the Building Inspector.
3. Prior to the issuance of a 'Building Permit" the owner of any lot to be serviced by a new
driveway shall apply to the BPW for a "Driveway Permit" by completing the pertinent
portions of the permit (see attached). Once the location of the driveway is approved by the
BPW, a building permit may be issued.
4. The Building Inspector shall not issue an "Occupancy Permit" unless the driveway to the lot
has been approved by the BPW.
5. By the issuance of a driveway permit for the stated location, neither the City of Northampton
nor the Department of Public Works imply that no drainage problems will result with the
driveway when constructed. Properties situated or driveways installed in low lying areas in
the path of the natural drainage will be subject to water problems. These problems may
include water sheeting across roadways adjacent to the driveway. The City and the DPW
assume no responsibility for any such drainage problems. The owner of the property is
responsible for constructing and maintaining the driveway with adequate provision for natural
water runoff situations.

as supervisor.CMR 780 Sixth Edition Section 108.3.5.1.
