The undersigned agrees to perform the following work for the Pioneer Valley Transit
Authority, at its Northampton Facility at a total cost of $
SPECIFICATIONS:
1. Remove loam as necessary from a ground section of eighteen (18)foot by
seventy-five (75) foot section, plus an irregular extended area sufficient to
joint with curve in front drive, making newly paved area square with existing
driveway.
2. Remove subsoil and lay fabric. Provide substrata of a minimum of eighteen
(18) inches of gravel, filled and spread.
3. Overlay with two and one-half(2.5) inches of base blacktop and cover this
base with minimum of one and one-half(1.5) inches of state grade asphalt
topping. Asphalt surface shall be level with existing drive and pitched
southward at a slope pitch of a minimum of 3 inches.
4. Backfill with new shoulders on three sides to a minimum of three (3) foot and
cover with minimum of two inches of loam, seeded and fertilized.
5. All work shall be completed within three (3)weeks of notification of bid
award.
6. Permits and utility clearances for any construction shall be the responsibility
of the Pioneer Valley Transit Authority contractor.

Dear Ms. Misch:
Please issue a permit for fill and paving work at 54 Industrial Drive, Northampton, MA
as shown in the attached documentation and the specifications outlined in the bid
proposal form. The purpose is to construct ten (10) additional parking spaces,
designed to be used for Pioneer Valley Transit Authority contractor vans. These vans
are used for para-transit services in the surrounding community area.
Our current parking situation has become critical as services have expanded.
Additional service vehicles and staff requirements have created a need for additional
spaces on our facility parking area. The proposed project should alleviate much of that
problem.
The proposed work project will be co-joined with an existing double lane exit drive. The
added pavement area will be off the street and will have no city street curbing or
contact. Two existing small trees will remain and aid in screening the new parking
spaces somewhat from the street. No trees or other shrubbery will be removed as a
result of this construction project. No utility lines, above or below ground, are expected
to be affected, to our knowledge. The required percentage of open area, specified for
the size of the lot, will be retained, including an open strip between our property and the
adjacent property.

I am writing today in order to request a permit for improvement upon our property. We
would like to add parking room for some of our vehicles along the side of our building.
The area in question measures 60' in length, 25' in width, and about 2' in depth. The
depth will be filled with processed gravel, and we would also like to add about 2" of
stone dust on top.
Our maintenance supervisor, Zbigniew Tur, will be in charge of overseeing the
construction process, which should take about a month once we have contracted a
company to assist.
