25A-188 (16) Conditions: Driveway Permit
In lieu of plan approved by City Engineer
I agree to the following added conditions:
1) I will contact the Department of Public Works and have
an inspector check and approve the graded gravel base
prior to paving to insure compliance with slope and
location;
2) I further agree that if in the inspections any of the
permit conditions are not met that I will at no expense
to the City remove and replace the driveway as directed
by the City Engineer.

plan showing the proposed driveway with grades and location
in the future to avoid possible expense which you will incur
by not getting approval of actual plans in advance.

Dear Ted,
Enclosed, please find a copy of the curb cut authorization
issued to us from the D.P.W. As you can see they have only all-
owed us 15 ' of cut. Is there anything that can be done to allow
a 24 ' cut for trailer trucks? We are also enclosing a copy of our
agreement with the engineer of record, Mr. Charles McDonald to
perform to the regulations of section 127 of the state building
code.
We will also be submitting the required mechanical drawings
as soon as they become available to us. Thank you for the time
you spent with me regarding this project.
