Dear Mr. Vafiadis :
On June 1 , 1995 I observed three eyewash units that were
installed by Coca Cola unlicensed workers . This is a violation
of Massachusetts State Plumbing Code 248 CMR 2 . 03 .
You are ordered to hire a licensed plumber to obtain a permit for
removal of the piping , fittings , valves , tee-in main and the
eyewash stations . The licensed plumber must also include the
reinstallation on a Massachusetts approved Plumbing Application
Form.
These violations are all subject to fines by the Commonwealth of
Massachusetts . This order of violations will remain in the
active file in this office .
