In accordance with the Sixth Edition of the Massachusetts State Building Code (780 CMR) §116, I
hereby certify that I have personally prepared the STRUCTURAL design drawing S 1 ( dated
2/26/08) for the above-named project. To the best of my knowledge this drawing conforms to the
applicable provisions of the Code and to standard engineering practice.
I shall also personally perform the necessary professional services during construction of the above-
mentioned portion(s) of the project to determine that such construction satisfies the design intent of
the calculations, drawings and specifications and the documents approved for the building permit,
including:
1 Be present at the site at appropriate intervals during construction to become generally
familiar with the progress and quality of the work and to determine in general if said
work is being performed in a manner that is consistent with the design documents.
I shall also submit to you a final report as to the satisfactory completion of the above-mentioned
portion of the construction work.
