Industrial Drive Northampton, MA 01060 Re: Coca Cola Dear Ray, I inspected the site at 45 Industrial Drive new addition on Friday February 24 at approximately 12:15 PM. The north area section up to WD.5 had been poured that morning. A follow-up inspection was done on Tuesday, February 28 at approximately 7:10 AM at which time I found said section exposed and not blanketed or protected from cold (temperatures over the weekend were in single digits and there was an ice storm on Monday) . I informed the site forman that this procedure was in violation of State Building Code articles 1504.7 and 1504.7.1 and Coca Cola Spec Book for cold weather protection section 1.05 article A. I inspected the site north area section WD.5 on Wednesday March 1, 1995 and found forms set and ready to be poured. The footings were cover- ed with approximately 4" to 2" of ice and rebar coated with ice. I inform- ed site forman that this was unacceptable and I would not approve this section of wall. The pour was stopped, ice removed and forms were covered. I inspected the site at approximately 6:45 AM March 2, 1995 and approved forms, footings and rebar, concrete was poured approximately 10 AM. I must state I am concerned that because of "fast track" time table that proper protection against cold weather, ice and snow are not being followed. This has forced me to the site daily in both the AM and PM to assure that the code is followed. Sincerely, Anthony L. Here is the status of local permits: Building Inspector: A temporary Certificate of Occupancy needs to be issued before production begins. (A full C.O. will be issued when the warehouse is completed.) 1. Final Electrical check list items should be completed by today. 2. The Fire Chief has done his walk-through. (He suggests that, if they install a direct line to the Station, the insurance premium would be reduced. They plan on having the fire alarms ring in the plant where a security person will call the Fire Dept.) 3. OPD needs to indicate that Special Permit conditions have been met. (We are waiting to receive materials from Ray Remillard) Department of Public Works: 1. Sam is reviewing the Agreement between City and COKE. We are awaiting response from COKE re: the Agreement. It should be signed before production begins. 2. Sewer Connection Permit: Ray Remillard needs to submit to Sam a revised Permit application. Sam will approve it and send it to DEP. There is no need to await DEP response; COKE can commence operations. 3. Discharge Permit for Treatment Plant: DPW will issue a permit to COKE. Sam is holding up on this until the Agreement is signed. He can issue the permit for a 30, 60, 90 etc. day period if it is deemed appropriate to commence operations prior to having the Agreement fully signed. He will await further discussion on this.
