application(s) for approval for new product(s) as indicated on the attached list.
Approvals
The approvals shown have been granted subject to installation in compliance with CMR 248,
Massachusetts State Plumbing Code and/or the Massachusetts Fuel Gas Code; said
approvals are in effect for one year beginning May 5, 1999. At the expiration of the provisional
one year approvals it will be necessary,for the manufacturer to petition this Board for an
extension of said approvals. You are further advised that the preceding approvals are not to
be construed as an endorsement of these products nor is this letter to be used or reproduced
as advertisement of the products.
Tablings
If a product has been tabled 'Pending receipt;of(specified)fee', remit the balance of fees due
directly to the Board Office at the address shown above. If this is the only reason for tabling,
you will not be required to appear before-the Board again
If a product has been tabled for'Failure appear as required', call the Board Office at (617)
727-9952 to schedule an appearance,before the Board's Subcommittee. Failure to reschedule
within six months of the date of this letter will!result in application fee forfeiture and the closing
of your file.
If a product has been tabled for any other reason, take whatever action is necessary to correct
the problem. Only then should you contact the Board Office to schedule an appearance before
the Board's Subcommittee.
