I agree to the following added conditions:
1) I will contact the Department of Public Works and have
an inspector check and approve the graded gravel base
prior to paving to insure compliance with slope and
location;
2) I further agree that if in the inspections any of the
permit conditions are not met that I will at no expense
to the City remove and replace the driveway as directed
by the City Engineer.

GENERAL INSTRUCTIONS AND REGULATIONS
1. A "Driveway Permit" is needed in all cases where a new curb
cut, or an alteration to an ezisting curb cut, is proposed
on a city public way.
2. Said "Driveway Permit" issued by the Board of ?ublic Worss
(B.P.W.) , shall be attached to and become part of the "Build-
ing Permit" issued by the Building Inspector.
3. Prior to the issuance of a "Building Permit" the owner of any
lot to be serviced by a new driveway shall apply to zbe B.P.W.
for a "Driveway Permit" by completing the pertinent portions
of said permit (attached) . Once the location of said driveway
is approved by the B:.P.W.a build:!=g permit may be issued.
4. The Building Inspector shall issue ao "Occupancy Perri-" unless
the driveway to said Lot has been approved by the B.:.W.
