Administrator to the Board,
A modular home Built by Avis Homes was delivered to the above
address in our city. The following are a list of violations that
were found:
1. Serial numbers for state approval labels not consecutive.
(R3 . 3 . 4)
2 . MSDS sheet supplied by manufacture showed that Bow Solder
used, a check with product approval from Plumbing Board of
Examiners shows product approval expired April 1st. 1990.
3 . Water tightness test asked by Plumbing Inspector failed.
4 . Toilet located on second floor supplied with hot water
instead of cold, piping crossed.
5. Garbage disposal wired improperly (Chapt 3 - 527 CMR) .
6. Piping schematic supplied did not match unit shipped.
The contractor and plumber hired by contractor repaired the
plumbing violations and when the leak was repaired the plumber
found that the waste line on second floor had two 90 degree joints
installed without clean outs, a second floor tub unit drain had not
been piped at all, and toilet on second floor had not been vented
to code.
The Plumbing Board of Examiners and Board of Examiners of
Electricans (the authorities of control of this specialized codes)
will be notified in writing of these violations.
The state approval numbers were 59496, 59497 , 59498, 59420. PFS
approval number was 306425, state system approval number was MCO27 .
A occupancy permit was issued 6/16/97 after corrections were made
by contractor.
