SUBJECT: Revised Site Plan—Septic System Const. Permit Lot 5B Woodland Dr.
The Board of Health is in receipt of a revised site plan, from builder John Zieminski, for the
proposed home construction project on lot 5B, Woodland Drive, Florence. The revision has
the dwelling and driveway slightly relocated with no change in the location of the leach field.
The Board of Health approves of the proposed revision in relation to the previously approved
leaching system plan, #30-97. Upon installation, the design engineer will provide an "as
built"plan, which indicates the location of the septic system and the dwelling.
