Once a criminal complaint is issued, the District Attorney's Office is
responsible for prosecuting the case. Because you are the complainant in this
matter, we depend on you to provide the District Attorney's Office with the
information we need to prepare this case.
Attached you will find a witness statement and restitution form. It is your
responsibility to fully complete both forms. (Please list any witnesses names and
addresses completely; attach copies only of receipts, pictures, etc.) Do not attach
originals. Please keep these with your files. You will be notified if and when the
originals are necessary. If you change your address and phone number, it is your
responsibility to notify us.
This information must be made available to the District Attorney's Office at
least two weeks prior to the arraignment date. It can be mailed or hand-delivered to
the District Attorney's Office located at 1 Gleason Plaza, Northampton, MA,
01060. Please indicate the docket number and arraignment date in the space
provided.
