POST MEETING ITEMS:
1. John D.,Jebb D.,and Bill T.marked out the location for the concrete pad for the air cooled condensing
unit on the ground adjacent to the air shaft. It was coordinated with the future MRI project.
2. John D. & Bill T. reviewed the location of the two air cooled condensing units in the air shaft.
Locations acceptable.
3. Bill T. & John D. discussed locations of concrete pads below exhaust fans. Locations acceptable.
4. Bill T. discussed with Bob A. and John D. the importance of cutting the wall to enlarge the louver
opening to serve air handling units S-1 & B-1. This has to be done as soon as possible so that S-1
can be installed, piped, and wired.

14. Jim M. from MJM reviewed the safety requirements and stressed the importance of wearing hard hats
and adherence to OSHA requirements.
15. Jim M. from MJM requested all his contractors to give him the number of men that will be working
during shutdown so the cafeteria personnel can have an idea of the number of people they have to
serve.
16. The maintenance shop area will be designated as a temporary break area for all workers. Jim M.
indicated that all workers will have to take a break at the same time in the morning and afternoon.
17. Bill T. and Ernest M. reviewed the drawings and specifications with the building inspector. He put
the project under controlled construction and indicated he will make periodic field inspections.
18. Yankee Technology should have a representation at all future meetings.
19. KS has taken field measurements of most of the ductwork in basement and is presently fabricating
ductwork at their shop.
20. Project is progressing on schedule with no major problem to date.

1. Bill T. reviewed the preconstruction agenda with the contractors.
2. Bill T. indicated job meetings will be held on Wednesday at 8:00 AM in the engineering conference
room for the duration of the project. As we approach the shutdown period, more meetings will be
scheduled.
3. Contractors did not anticipate any delivery problems with major equipment.
4. Bill T.stressed the importance of maintaining a record set of as built drawings. Contractors will make
as built changes as they proceed with the job rather than making them at the end.
5. Jim M.for MJM will conduct a simulated noise demonstration as requested by the Hospital at 7:00 AM
Thursday, July 9. All sub contractors shall take part in this demonstration.
6. Jebb D. will make a new reflected ceiling plan indicating the location of the light fixtures to agree
with our electrical drawings.
7. New OR lights will be installed in OR # 3 & 4. Light from OR #3 to be reinstalled in cysto OR.
8. Urist:ut contractor will attend one of our meetings in July to become familiar with the project and
be aware of the time frame.
9. Bill T. will give the smoke detector locations in the ductwork to Collins Electric.
10. Mitch K. will give the push button control location in the nurses station to C.E.
11. Drop cord locations in the Cysto OR to remain.
12. Mitch K. will review the height and location of switches in the OR's with Jim M. from C.E.
13. Jim M.from MJM submitted a draft of the project schedule to all the contractors. Before it is finalized,
all contractors will give Jim M. their input.

12. Jim M. will have the insulating contractor attend the next meeting.
13. A shutdown on the medical exhaust system is required for approximately 4 hours. John D. will
coordinate with engineering department.
14. The engine exhaust pipe from the cutting machine will have to be extended away from the fresh air
intake louvers on the areaway to prevent exhaust fumes from entering the intake. Bob A. will
coordinate.
15. A conduit for the existing temperature system has to be relocated that feeds the Andover controls.
Dan M. from YT will try to determine what areas it controls. Shutdown time is about 3 hours.
16. Next job meeting is Wednesday, July 15 at 5:00 A.M.

4. The push button location in the nurses' station has been finalized.
5. All contractors have to review their schedules during the shutdown time and give the information to
Jim M. so he can finalize the schedule.
6. Pete K. from KS indicated all the ductwork in the basement is field measured and about 90% of all
the ductwork is fabricated. During the shutdown period, KS will set up a temporary shop on the site
to fabricate ductwor c. as required.
7. Air handling unit S-1 will be moved into place on 7/9.
8. Bob Aquadro from AC is planning to cut in the new louver opening in the areaway on Saturday 9/11
and Sunday 9/12.
9. MJM will begin piping air handling unit S-1 on Monday 7/13.
10. The existing ventilating unit will be taken out of service on 7/9 to make space for S-1. Temporary
ductwork will be installed to provide ventilation to the medical records area and other basement areas.
11. A 1-hour fire rating has to be maintained between the corridor walls and the operating rooms. Jebb D.
from Al and Bob A. will discuss the method of fireproofing at all wall penetrations above the ceiling.

the support system for the new operating room light fixtures.
11. The hospital engineering department will determine if the ceiling mounted medical gas delivery system
and the related piping can be removed in the new ortho OR.
12. Bill T. informed Bill S. from JA that a shop drawing is required for the sealing material between the
light fixtures and the ceiling grid. Bill will furnish for the next meeting.
13. Project is progressing on schedule with no major problems. Delivery of equipment is on schedule
to date.
14. Next project meeting is Wednesday, July 22 at 8:00 A.M.

1. Bill T. will furnish John D. from MJM BTU input to preheat coils so that proper steam traps can be
installed.
2. The steam preheat coils on S-1 and B-1 have to be removed so that the freezestat capiliary can be
installed. Dan from YT will install the freezestat controls.
3. I informed Pete K. from KS that a volume damper has to be installed upstream of the motor-operated
damper in the fresh air duct to B-1.

1. Jebb D. and Bob A. agreed that a sprayed-on fireproofing material will be applied on the corridor
side to seal all wall penetrations above the corridor ceiling.
2. Bill T. will make 4 copies of the reflected ceiling plan and give to Jim M.
3. Next Wednesday(July 22)is the deadline for all subcontractors to submit their schedules for shutdown
to Jim M. He needs it by the 22nd so he can finalize the schedule and coordinate with hospital
personnel.
4. The new fresh air intake opening has been cut in the areaway to serve S-1 and B-1. The opening
is temporarily covered with plywood.
5. The new chilled water main's serving S-1 and B-1 have been installed. The taps into the existing
chilled water mains have been made.
6. Dan M. from YT is in the process of relocating the conduit that feeds the Andover controls. No
shutdown is required. The conduit has to be relocated to avoid a new supply duct and wall opening.
7. Jim M. from MJM indicated spare parts for the new air handling units could be available in a few
days if required.
8. It will be determined by the next meeting if 9'-0"ceiling heights are acceptable in the operating rooms.
9. Jim M. from MJM has not been able to x-ray locations of new floor penetrations and will try next
week.
10. Operating room light fixture status: OR#1 existing light to remain; OR#2 existing light to remain,
OR#3 new light to be installed Sunday, August 9th, approximately 4 hours installation time; OR#4
new light to be installed also on Sunday,August 9th,approximately 5 hours installation time. Existing

1. All subcontractors have submitted their work schedules to Jim M. for the shutdown period. Jim M.
will finalize the schedule this week and be prepared to discuss it at our next meeting.
2. A representative from the operating room light manufacturer will be present at the next meeting to
discuss minimum ceiling heights.
3. The ceiling-mounted medical gas delivery system in the new ortho OR will be removed by the hospital
maintenance staff within the next two weeks. This will give the sheetmetal contractor additional ceiling
space to install the supply and return ductwork.
4. Yankee Tech will be permitted in the ORs on Friday, August 7" to remove the existing controls in
each room. However, personnel have to scrub while working in the ORs.
5. All locker rooms will be open to the contractors of Friday, August 7"' for them to start work.
6. Bill B.from CE requested a final approval for the drop card locations in the cysto OR. Location should
be marked on the drawings.
7. Old ortho will have no x-ray equipment installed.
8. The push plate locations have been approved by the hospital staff.
9. Jim Moran informed all contractors to store equipment on site as soon as possible before shutdown.
10. Jim M.plans to x-ray floors at locations of duct penetrations to determine if electrical conduit is buried
in the floor slab. Work is tentatively scheduled for Saturday, August 1"and 2"d. Holes will be cut
through the corridor walls where possible.
11. Contractors will make every effort to keep the ceiling heights in the operating rooms at a maximum
height. In no case will they be lower than 9'-0" and 9'-2" in the new ortho OR.
12. The job meeting for August 51" will start at 9:00 A.M., not 8:00 A.M.

10. The road behind the future MRI will be closed down on Thursday, August 6th. Bob A. will move
in dumpster on Friday, August 7th. The road will be partitioned so that is could be used for an
emergency.
11. The coding alarm system for hospital emergency cases will be in effect during shutdown. Ernest M.
and Jim M. will coordinate locations.
12. Ernie M. sent shop drawings of the air handling units and exhaust fans to the balancing contractor.
The balancing contractor will be available to balance the system when the units are all piped and the
ductwork is installed.
13. Bill T. received shop drawings of the refrigerant piping. He will review with Ernie M. and Frank F.
before sending back to the contractor.
14. Bill T. informed Jim M. that all refrigerant piping shall be run in ACR copper tubing and the piping
will be charged with dry nitrogen while soldering the joints as specified on page 7.
15. All contractors are to notify their workers that smoking is not permitted in the hospital. All smoking
has to be outside the building.
16. Bob A. is planning to install dust partitions on Friday, August 7th and start cutting the holes in the
floors and walls on Saturday,August 8th. Bob A.will start in ORs#3 and#4 and move in the direction
of the support areas.
17. Yankee Tech will give a complete set of temperature control shop drawings to the Hospital engineering
staff before shutdown so they can review.
18. Jebb D. and Bob A. will investigate the best way to fire stop wall penetrations above the ceiling.
Decisions will be made before the next meeting.
19. Project is progressing ahead of schedule with no major problems. Delivery of all equipment is on
schedule.

1. Jim M. from MJM will give a manpower schedule to the manager of the cafeteria so they can be
prepared to serve more meals. Workers will not be permitted in the cafeterias on Saturday and Sunday,
August 8th and 9th,because of the dust and dirt that will be generated during demolition. Monday,
August 10th the cafeteria will be open to the workers.
2. The maintenance shop area will be designated as a temporary break area during shutdown. All workers
will have to take a break at the same time. Morning break,9:15;lunch break,11:15—11:45;afternoon
break, 1:45.
3. After a lengthy discussion with John Moore from the Berchtold Corp., it was decided by all present
that the ceiling heights in OR#3 and OR#4 would be set at a minimum of 9'-0". Lights in OR#3 and
OR#4 will be installed on August 17th. It should only take one day to install.
4. Jim M. from MJM will check the ceiling condition in the old ortho on July 29th.
5. Meeting schedule for week of August 3rd:
A. Ernest M.,Jim M.,and Bill T.will meet on Monday,August 3rd,to discuss the shutdown schedule.
Time: 8:00 A.M.
B. Normal project meeting on Wednesday, August 5th. Time: 9:00 A.M. (note change in time).
C. Last meeting before shutdown on Friday, August 7th. Time: 8:00 A.M. All contractors have to
be present for final coordination.
6. During shutdown, all contractors may be asked to attend daily coordination meetings. Jim M.,
Ernest M., and Bill T. will determine if and when they are necessary.
7. It is important that all contractors keep a record set of as-built drawings.
8. Ernest M. and Jebb D. will check with the building inspector when he can issue a temporary permit
for ORs 1 through 4.
9. Jim M.is planning to x-ray floors at duct penetrations on Saturday,August 1st and Sunday,August 2nd.
12. Bill T. and Jebb D. sent the certificate of substantial completion form for Phase I, OR#1 through#4,
to Ernie M.
13. The project is about 80% complete with no major problems. The cooperation between the hospital
staff, the contractors and the engineer has been outstanding.
14. The next scheduled meeting will be on Wednesday, August 26th, at 8:00 A.M.

1. Jim M. will complete the ortho wing by midnight, Sunday, August 23rd, about one week ahead of
schedule. Some minor work will be done in the basement the week of the 24th.
2. The variable speed drives controlling S-2 and E-2 will be set up by a factor representative on Thursday,
August 20th.
3. Don M. from YT will calibrate the thermostats and humidistats in the next few days.
4. On August 18th, Bill T. informed Jim M. to turn on the steam preheat and reheat controls to provide
heat for about 8 hours as indicated on page 16 of our specifications. No problem with control system.
5. Bill T.will call the York representative to determine the status of the low ambient control for the small
air-cooled condensing unit.
6. Jebb D. suggested that Jim M. video the ortho wing in more detail to verify the as-built condition
because of the possibility of future work in this area.
7. Jebb D. & Ernie M. will conduct a final inspection August 19th and start a punch list for architectural
items.
8. Bill T. will conduct a final inspection and start a punch list the week of August 24th.
9. Frank F.documented as-built conditions by photographing all critical areas. He will identify all pictures
and put in the maintenance file.
10. The balancing contractor is in the process of balancing ORs#1 then 4 served by S-1 and E-1. He expects
to finish Friday, August 21st. He will start balancing the ortho wing and support areas on Monday,
August 24th.
11. MJM's crew is piping the existing air handling unit and adding the"DX"cooling coil that will service
the support area.
AUGUST 7, 1992 —JOB MEETING #7 (FINAL MEETING BEFORE SHUTDOWN)
1. Jim M. reviewed the shutdown schedule with all the subcontractors.
2. For security reasons, all workers shall have ID badges.
3. Willie from KS will salvage AHU#8,steam reheat coils and humidifiers. Ernest M.will indicate where
equipment is to be stored.
4. Bill T.informed Jim M.that after all refrigerant piping is installed,it has to be purged with high velocity
nitrogen for about 30 seconds.
5. A vacuum-assisted condensate pump serving S-1,S-2,&B-1 has to be added to the condendate system
to prevent the remote possibility of the steam preheat coils freezing. Bill T.gave the model#to Jim M.
so he could order the pump. When installed,the pump will be the main source of condensate removal
from the preheat coils with a bypass to the gravity system. Bill T.will furnish Jim M.a piping diagram.
6. Bill T. will coordinate with Ernest M. and Jim M. when the balancing contractor should start.
7. KS has most of the ductwork fabricated and is waiting for the ceilings to be removed in the OR suite
to start installing the ductwork.
8. The variable speed controls have to be set up initially by a manufacturer rep. Mitch K.will coordinate.
9. The scheduled project meeting is on hold for next Wednesday.
GENERAL COMMENT:
THE COOPERATION FROM M.J.MORAN,THE SUBCONTRACTORS,THE HOSPITAL ENGINEERING
STAFF, AND THE HOSPITAL SURGICAL STAFF HAS BEEN OUTSTANDING TO DATE. LET'S ALL
KEEP THIS SPIRIT OF COOPERATION THROUGH THIS CRITICAL SHUTDOWN PERIOD.
To the best of my knowledge and belief, these meeting minutes are considered to be a true and accurate
record of all items discussed. The undersigned shall be notified,in writing, within ten (10)days from the
date above of any corrections or additions;otherwise these minutes shall be considered accepted as written.

15. Hospital security is an important concern. The contractors are to provide security to their own
equipment and an honor system regarding hospital equipment. None of the construction crew
permitted to use hospital equipment or material. Please be aware of the issue that this building is
a hospital.

OSHA.

1. No workers in cafeteria the first Saturday and Sunday. For the first week individual crews will break

3. Status of new ortho: attempt to make whole ceiling at 9'-T'. An additional soffit may need to be

4. Shut down about 3:00 p.m. on Friday, August 7th. Some work may be done in non-vital areas by
8:00 a.m. In OR's #1 & #4, non-aggressive demolition can start by 8:00 a.m.
5. Air handling units 7 & 8 must remain operational until 3:00.
6. All worker sill have to "suit up" until 3:00 on Friday.
7. Meeting for all contractors on Friday at 8:00 a.m.
8. The amount of noise Saturday a.m. (early) by wall demolition should be reasonable.
9. Daily meetings to be coordinated as the work proceeds.
10. The building code regarding the temporary occupancy permit: August 13th through the 17th (21st
at the latest). J.D. of Al will make appointments.
11. Regarding x-raying the floor: cancelled due to excessive exposure. Immediate review of possible
problems has provided a reasonable acceptance to proceed with little or no consequences.
12. Access to staging area next to dumpsters will be allowed only to unloading material. Parking for
crews will be behind McCallum Building, etc.
13. Paging/alarm protocol: workers are to report to the break room and wear a sticker tag with their
contractor name and date. The speakers will have to remain functional during construction.
