The red markings indicate what we believe to be the appropriate separations of the existing campus into three separate buildings. I have also completed the data forms which you requested for each of these buildings. I would now like to set up a meeting which would include yourself, our local inspector, Prank Sienkiewicz, and our consultants so that we may,discuss this infonnation and reach an agreement regarding the correction of any issues that need to be addressed. 1 would also appreciate your assistance in insuring that our flew AIRI/CT unit is able to go on-line as soon as possible. We have requested an inspection front DPII for the week of September 18, 1995. This date is yet to be confirned by DPH but I expect confirmation shortly. As you are aware, the DPH will not conduct this inspection without a current certificate from your department. Comments: The windings should be cleaned of dust and other foreign materials prior to the test. It should be conducted on a one— to three—year interval. B95-1-2 A short—circuit study should be performed to determine whether the current protective equipment is adequate. Comments: This facility has no record of a short—circuit study being performed in the past. There is a combination of old electrical equipment and new equipment that has been added over the years. This includes a significant amount of electrical load that has been added to the distribution system. A short—circuit study should be performed to determine if the current protective relays and switchgear are adequate to prevent a high current electrical fault that could cause damage to the distribution system. Mr. Margeson is aware of the need for this study and will look into having it performed during an expansion of the facility, which is planned in the near future. IMPORTANT CHANGES There is currently no up—to—date one—line electrical diagram available for this facility. Mr. Margeson is in the process of arranging for the completion of an up—to—date one—line and it should be completed in the near future. Future plans are to build an addition to the facility within the next year that will add bed space. This would be a good opportunity to complete an electrical one—line diagram and short—circuit study, as electrical engineers will be on—site during the expansion. The underside of this roof is not easily accessible, and the blueprints of the building could not be located. Comments: A Factory Mutual Red Tag Permit Kit was left with Mr. Margeson who agreed that it would be used. 95-4-4 A written Hot Work Permit should be used to supervise each hot work operation conducted outside the maintenance shop. Comments: Hot work operations are adequately supervised but a written permit is not used. Mr. Scott Johnson reviews all hot work prior to it being done. A supply of permits was left with him and will be used. 95-1-4 The roof perimeter flashing of the West Wing should be inspected and resecured in areas where it has become detached from the hook strip. The guidelines in the Factory Mutual Roof Fix It Guide should be followed (left with Mr. Margeson.) Comments: Much of the flashing of the West Wing has become detached from the hook strip. The flashing is the primary protection against wind damage to the roof covering. A roofing contractor will be contacted within a couple weeks to fasten this flashing. IMPORTANT CHANGES A small addition is under construction at the west side of the east wing. This is to be occupied by MRI (Magnetic Resonance Imaging) equipment. Full automatic sprinkler protection is planned, except in the gantry room where the electronic equipment and the patient are located. This room will be protected by a gaseous protection system similar to the Cat Scan room, which is presently protected by a halon system. At this time,only foundation work is underway. There exists a long range plan for renovation of many areas and relocation of some departments. All renovation plans include complete automatic sprinkler protection for renovated areas. Dry pipe valves are presently being trip tested quarterly with complete flooding of the systems. It was recommended that this frequency be reduced to annually and that the trip testing be done without flooding the systems. This will reduce corrosion inside the piping and will reduce the chances of water accumulations freezing in the winter. The Ethylene Oxide operation in the Main Building was reviewed with operating personnel and found to have satisfactory safeguards against physical property damage. The ETO mixture is 12 percent ETO and 88 percent inert, which is a nonflammable mixture. Appropriate interlocks are provided to ensure that ventilation running and that the doors are not opened during operation. The room is equipped with automatic sprinklers. (95-1-7) The operation of the wood chip fired boiler was reviewed with Mr. Donald Spear, Chief Engineer, and found to be satisfactory. (95-1-5) -2— Index 10965.06 RECOMMENDATIONS 95-4-1 Automatic sprinkler protection on an ordinary hazard pipe schedule o hydraulically designed to provide a density of.15 gpmisq. ft. over 2500 sq. ft. should be installed in the basement storage area of the Radiology Building and the adjacent area at the end of the corridor to the East Building where linens are stored. Comments: This 6,000 sq. ft. area is used for the storage of linens and medical records (paper files on metal shelving). A fire in this area would be very difficult tc control since it is in a below grade space, and access for manual fire fighting is very limited. Smoke damage to much of the hospital would be extensive. Mr. Ernest Margeson, Director of Facilities, supports the recommendation to install sprinklers i this area. Funding would need to be requested in the budget (fiscal year begins Oct. 1). Both Mr. Margeson and Ms. Debra Simmonds, Risk Manager,indicated that several agencies are inspecting the hospital for fire safety items and each has recommendations. The reports from all agencies must be taken into consideration when prioritizing expenditure of resources. 95-4-2 All main supply valves on the fire protection, , and systems should be clearly labeled, and personnel on all shifts should bet trained n their d water location and operation. In the event of a piping failure prompt action by personnel present car reduce damage considerably. Comments: Liquid damage is one of the primary causes of loss at hospitals. There are maintenance personnel on the first shift only at this facility. Some basic training on the location of shutoffs and labeling them could enable security personnel or others trained in these actions to shut off water if an incident occurred during the off shifts. Both Mr. Margeson and Mr. Kurt Peterson, Maintenance Supervisor, agreed that improvements are needed in this area and had previously been planning to provide additional training to the security personnel. Additional labeling of valves will also be done. 95-1-1 All valves controlling water supplies to automatic sprinklers should be locked open with nonbreakabZ shackle locks and sturdy chains. Comments: This will be done. It was agreed that, if a valve were in a secured area and equipped with a tamper switch, the lock could be omitted if desired. All post indicator valves outside are presently locked. 95-1-2 A fire prevention inspection program should be implemented. This should include a weekly visual inspection of all sprinkler valves and dry pipe sprinkler system air pressures. Any deficiencies in housekeeping and extinguisher conditions should be noted. On a monthly basis, the post indicator valves on the sprinkler systems should be unlocked and physically checked for the open position. Comments: This will be done. Several sample forms were left with Mr. Margeson and the recommended inspection program and frequencies thoroughly discussed. Please advise where future reports should be sent. 150 years Over one hundred and fifty years of orogress and stability The following drawings were provided to us by P.R. Sherman, P.E. Of Woburn, Ma. They indicate our stair and elevator terminology and smoke separations of the facility by floor. Lastly you will find a report by our Factory Mutual Insurance Inspector, Jim Burnham. This inspection was completed in May of 1995. In general, rated stairs, shafts, and elevators are located properly but require both priority and routine work. Occupancy separations and smoke barriers require routine repairs. Corridors in many areas do not provide required fire resistance and do not have rated doors. In lieu of upgrading the fire ratings, a large number of problems will be solved by the installation of sprinklers. Hazardous areas require both upgrading of separation walls and, in some cases, the installation of sprinklers. Fire dampers need to be installed in several locations. These situations require correction with a priority given to patient care areas. Several areas show suspect ceiling tiles which should tested for flame spread. The egress system consists of exit access paths which lead to smoke barriers and thence to exits. Some dead ends are present. Most are not significant problems but situations on West 1 and Main 3 should be corrected by means of changes to security procedures. Several areas are open to corridors, which should be corrected, for instance waiting and storage areas in OPD. These corrections involve changes to sprinkler heads and upgrading detection systems. Some stairs are not in compliance with regard to riser height, width and handrails. Geometric problems will be corrected as part of larger projects, or a life safety equivalency will be applied for. Stair discharge issues can be resolved with signage and hardware. In addition, there are several issues with exit signs, door hardware, restrictions to corridor width and other minor issues. Sprinklers have been installed during recent renovations. This has corrected some previous problems throughout the facility. Additional sprinklers will be installed as part of ongoing renovations. This will be the solution for some corridor rating and hazardous area rating problems. Some existing sprinkler head locations are improper and require correction. We have been checking our smoke and fire seperations for penetrations and repairing all those found. This process will be ongoing for the next few months. In general, the problems found are typical of hose found in facilities of this type. Recent renovations have corrected problems in renovated areas and upcoming renovations can correct some other issues. Some problems will be solved by relocating patient care out of the South, East and Main Wings, which will occur at the latest by the end of 1998. A few issues are identified which will result in the expenditure of significant dollars, for instance the fire wall and sprinklers, but most can be fixed as part of ongoing operations and renovations. We all need to agree on the staging of repair by priority so that all interested parties are satisfied. During Project 2000 a complete new fire alarm system will solve many existing issues. We also have developed a six year budget to address life safety issues. It assesses the state of Cooley Dickinson Hospital in terms of the Life Safety Code, 1991 edition. This code is enforced in existing buildings by both the Joint Commission on Accreditation of Health Care Organizations (JCAHO) and the Massachusetts Department of Public Safety (DPS). Several violations of this code have been identified which will require correction. We felt that it is important to develop an approach to correction which is acceptable to both JCAHO and DPS. It is not the intent of this report to present detailed solutions for those issues which may require architectural design but, instead, to identify key issues in Life-Safety and separate buildings arto proper usage. This report deals only with the physical facility. It does not deal with operational testing or inspection requirements. The proposed addition will fall within the limits of the right-of-way which was retained by the City following the recent discontinuance of a portion of Denniston Place and Hospital Road. The successful discontinuance of Denniston Place as mentioned above will negate the City's need to retain said right-of-way and the hospital will be able to construct the proposed addition as shown. 3. Our records show that portions of the proposed parking lot located to the south of the Denniston Place Extension will be built on land owned by the Smith Vocational School. 4. The proposed drop-off areas located in front of the hospital entrances do not channel traffic flow efficiently. 5.
