Forsythe— FCS Cooley Dickinson Hospital 30 Locust Street Northhampton, MA 01060 Reference: Structural Engineering Peer Review New Surgery/Beds Expansion Project Cooley Dickinson Hospital Northampton, Massachusetts Dear Ms. Forsythe: As you have requested, we have conducted an independent structural engineering review of the foundations for the above-referenced project as prescribed by Section 110.11 of the Massachusetts State Building Code, Sixth Edition. The documents reviewed were as follows: 1. Structural Drawings S100, S200.A, S200.B, S201.A, S201.13, S210, S401 up to and including Addendum No. 4 dated 4/07/05 prepared by Struble Engineering, LLC. 2. Specification Section 03300 (Cast-in-Place-Concrete) dated 02/28/05 prepared by Struble Engineering, LLC. 3. Geotechnical engineering reports dated May 21, 2004 and March 11, 2005 prepared by Weber Engineering Associates, LLC. These documents were reviewed with respect to only the design of the foundations. The superstructure(s) and its connections to the foundations will be the subject of a separate review. FOUNDED IN 1959 BY RICHARD W.SOUZA AND EDWARD K.TRUE • Installing rebar for concrete • Encountered unsuitable soil at the footings for cooling tower, replaced with compacted fill, this will result in extra costs Note: • TRO observed that both stairs and connector concrete slabs had been placed, Barr & Barr was notified that concrete finish is unacceptable. Concrete to be removed and replaced at no cost to owner • TRO noted at 3` floor at E-3.4/4 there is a damaged steel tube column. Barr & Barr advised that they had informed Structural Engineer. The presence of such high residual stresses that are certainly the result of the plastic deformation of the tube walls lead me to reiterate my recommendation from my previous noenno, calling for removal and replacement of this particular post. Finally, | observed the installation of high CyNU partition xvoUo at the fourth floor mechanical area. These high partitions are tobe reinforced vertically and evidence of this reinforcement was noted in the CMU being laid up. At finished partitions under the roof deck, light-gauge bracing angles needed to be installed on alternating sides of the portitiono, in accordance with sketch A0003-15fronn Addendum No. 3. Since the new steel must match the elevation of the existing steel, | asked Steve De|isi of Barr& Barr to field-check the elevations before erecting steel. Also, | said | would expect some sort ofrevision to the previously-processed steel shop drawings that reflected recent scope changes to the cooling tower installation. Mr. C>e|ieiaaid he would investigate these items. | also field- measured the location of a 16"-diameter drain line under the existing cooling tower to see if its continuation into the new cooling tower would conflict with diagonal bracing o|aUad to be installed there. Later checking with drawing S-210 showed that conflict would bounlikely. This clearance will be checked again when revised shop drawings are reviewed. I viewed the installation of new and updated utility lines suspended from the existing chiller plant roof. | had previously investigated the effect of the weights of these lines on the existing structure and found them adequate for support, but one line under the original section of the plant could not be checked due kJ the absence ofexisting drawings. | observed this line tobe suspended from spring isolators hanging from spreader bars that hung from the top chords of the existing 24"-deep joists. At two locations, the hanging points for the spreaders were located between panel points of the joiSto, putting the top chord in banding. Such conditions are to be avoided Uz allow the joist to act ao a truss (axial forces only). | asked Mr. C)e|iai to add afie|d-mxe|ded angle strut from the top chord hangers to the nearest bottom chord panel points. He said he would ask the miscellaneous metal subcontractor 1odothis. | also took measurements of the unchecked joists' components and later analyzed their performance with the upgraded line load on them and found them tobeadequate. In the main building, | observed the damaged tubular post between lines 3.4 and 4 o line Eabove the third floor(reference rnyO4-21-O8 memorandum on this aubiect). The extent of outward deformation of the tube's vvaUo was greater than | had expected, extending up from the floor elevation more than 6', while not appearing at the shop- welded angle connection for the lower horizontal girt framing into the post approximately 2'-8^ above the third floor. This condition still seems to point to ice expansion within the post creating outward pressure that deformed the tube walls, but from a higher column of ice within the tube. Struble REFERENCE: Cooley Dickinson Hospital Surgery/Bed Expansion SUBJECT: Site Visit on May 9, 2006 DATE: May 10, 2006 ------------------------------------------------------------------------------ A site visit was made on May 9, 2006 to observe the progress of the structural work, primarily at the Chiller Building expansion, but also on the main building expansion, which is nearing completion, structurally. The foundation walls for the new chiller building addition had been cast and workers were installing underslab utility lines. The widths of these walls were measured at 16" while the structural drawings called for 15" widths. While not a structural concern for the function of the walls, the overall width of the addition is intended to match the width of the existing chiller plant building (45'-6"). Hence, this additional inch of wall width could reduce the clear distance within the addition by 2" and might reduce the lengths of the roof joists by the same amount. This is a coordination issue to be addressed in the equipment layout of the new space, the shop drawing submittal for the roof joists and the embedded hardware that is to be installed at the tops of the CMU walls. I notified the field personnel of Barr& Barr of this issue. The new cooling tower foundations had been cast and backfilled, with four piers in place ready to receive the new support steel framing for the tower. It appeared as if the distance between the tops of the piers and the top of the existing cooling tower support Site Work: None today due to weather Chiller Plant • Prep S.O.G. for concrete • CHW line work continues (welding); x-ray tests next week Note: • TRO reviewed metal panel drawings with subcontractor, maximum width of panels is 59" • TRO to re-layout canopy with new panel width in mind • TRO met with Fire Marshall to review exit sign locations and pull station locations • TRO reviewed Mockup of Patient Room (Bulletin to be issued to clarify owner revisions) • At low roof at south elevation replace all poly iso insulation along edge of roof exposed to weather REPORT BY: DC/rra Page 2 of 2 ARCHITECT: TRO/The Ritchie Organization CONSTRUCTION ADMINISTRATOR: Dana Cooper cc: Barr and Barr-Patty Eldridge and William Aquadro, CDH-Tim Singleton, Dale Taglienti, Stacey Yeragotelis, Keith Davis Dave Wolkowicz Giuseppe Colosimo Steve Delisi• Building Inspector AIA DOCUMENT G711 •ARCHITECT'S FIELD REPORT•OCTOBER 1972 EDITION•AIA• 1972THE AMERICAN INSTITUTE OF ARCHITECTS, 1735 NEW YORK AVE., NW;WASHINGTON, D.C. 20006 I NN No ARCHITECT'S FIELD REPORT PROJECT: Cooley Dickinson FIELD REPORT NO.: 21 CONTRACT: PCT ARCHITECT'S PROJECT NO.: 2323 DATE May 16,2006 TIME 11:00am WEATHER Rain TEMP.
