CONTROLLED CONSTRUCTION AFFIDAVIT:
In accordance with Section 116.0 of the Massachusetts State Building Code, 8th Edition, I
Charles P. Sharples, P.E., Massachusetts Registration No. 28940, begin a Registered Professional
Engineer, here by certify that I have prepared or directly supervised the proposed repair methods
for this project.
I further certify that I will perform the necessary professional services during construction and be
present on the construction site on a regular and periodic basis to determine that the work is
proceeding in accordance with the approved methods and specifications approved for the
building permit.
