Subject: Violation780 CMR State Building Code and Massachusetts Electrical Code
Mr. Ghalibaf,
I was called to the scene of a fire at the Hotel Northampton on August 11, 2006 at approx
10:30am. The fire located near rear entrance just out side of the kitchen area appears to
be caused by careless disposal of a cigarette in bark mulch which ignited. The mulch
burned thru the siding and started to ignite the wood framing creating large amounts of
smoke which entered the building and kitchen area. After the fire was extinguished an
inspection took place of the kitchen area adjacent to fire origin. Several serious violations
of the Massachusetts electrical code were found, specifically the use of extension cords
for permanent wiring. The area behind the flying equipment had a large wound up
extension cord covered in grease that was feeding one of the ovens (photo #1). Another
cord ran up the wall ,behind ceiling tiles around a water pipe and was feeding equipment
on food line (photo#2). Another wound up extension cord covered with grease was
located at far end of cooking line behind stove and it was also wrapped around the gas
cock,was supplying power to a oven(photo#3). A new fridge located near entrance to
kitchen was being powered by another extension cord that was behind fridge being
pinched by fridge against the wall (photo#4).
The use of extension cords for permanent viol violations s is a were ineareas covoered with overheating
rease and
causing fire, the locations of two of
presented an extreme hazard. The Hotel has been inspected various times, and on
November 23, 2004 the hotel was cited for the use of extension cords being used as
permanent wiring.
I am citing the Hotel of Northampton for violation of 780 CMR section 103.1 for failure
to maintain service equipment in a safe and sanitary condition. The violations were
corrected on August 11, 2006 and the Inspector of wires inspected and signed off on the
work performed.
This letter shall serve notice that you have violated 780CMR and could be subject to
