Dear Mr.Boulanger:
It has come to my attention that your fuel company has installed a gas boiler and indirect water heater at the
above referenced address. This installation has been done without plumbing or gas permits by your
installers. Also,there are numerous other State Plumbing and Gas Code Violations.
Gas Code Violations
1. No gas permit for basement boiler install.
2. No testing of new gas piping to boiler.
3. Installers not licensed plumbers for gas fitters.
4. Basement CO detector not installed must be hardwired.
5. 1"&2nd floor CO detectors not installed can be hard wired or battery operated.
6. No signage above exhaust at outside of building.
7. Improperly hung gas piping at boiler.
g. No inspections.
Plumbing Code Violations
1. No plumbing permit for the install of water heater and water piping to boiler and water heater.
2. Unlicensed personnel doing the installation.
3. No testing of the water pipe installation.
4. No setting of hot water temperature to the plumbing fixtures.
5. No inspections.
