5. Review/Approval Criteria
In conducting the Site Plan Review, the Planning Boax.d

The Planning Board shall use the criteria of Section
10 . 11 (5 . ) for approving/disapproving the Site Plan.

Application for Site Plan Review Special Permit shall be made
to the Building Inspector on forms provided for that purpose,
accompanied by the require fee. The Planning Board shall
adopt specific rules governing application and fee. When the
application has been received in a completed form as
designated by said rules, a copy shall be forwarded to the
City Clerk. The stamp of the City Clerk shall designate the
date of filing. Copies of the application shall be
distributed to those boards and departments as specified in
the aforementioned rules.
? In the case of a major or intermediate project which requires
a Special Permit as identified in Section 5 . 2 , Table of Use
Regulations (or other applicable sections of this Ordinance) ,
an additional copy of the above Site Plan Review application
shall be provided by the applicant at the same time

for a Special Permit.
The application for Site Plan Review shall be accompanied by
a site plan, drawings and supporting documentation in a form
specified by rules and regulations which shall show, among
other data, the following:

b. Site plan(s) at a scale of 1" = 40 ' (use more than one
sheet if necessary) , showing the following.
