8. A copy of a letter dated February 3, 2003, from the Planning Board Secretary, Angela
Dion, stating that she would record the order of conditions once the ten day appeal period
was expired. (I do not have a copy of this receipt,but I assume the Planning Board does.)
Although the final construction plans for the building are still being worked on, we are hoping to
be allowed to work on "foundation only" at this time for the project to stay on schedule. The
floor plans are complete, and the remaining architectural, mechanical, and fire alarm drawings
are getting underway. There will also be an energy code compliance narrative submitted. We
would like to start our foundations the week of April 28 if this timeframe works for you.
The final septic system design is being worked on by Tim Maginnis, since the location of the
system has moved on the site to accommodate the last round of changes. I expect to have copies
of this design soon and will be able to pass them along to you or others as needed.

2. Workers compensation insurance affidavit dated April 15, 2003.
3. Construction control document dated April 15,2003.
4. Check No. 021159 in the amount of$3,385.80 dated April 15, 2003. Also attached to the
check is the worksheet on how the permit fee was calculated.
5. Three sets of site plans SPR-1 through SPR-6 by the Berkshire Design Group, Inc., all
revised and dated April 7, 2003.
6. Three sets of building plans by The O'Leary Company including a cover sheet with code
review, floor plans A101 and A102,building elevations on A104, a progress print of wall
sections on A106 and A107, and foundation plans S101 through S106 with all plans
having an issue date of April 15, 2003.
7. A copy of the receipt dated April 2, 2003, for recording the Planning Board decision on
this project at the Hampshire County Registry of Deeds.
