Jonathan Fisher, Leah Fisher, Stephen Fisher and Phillip Fisher. Based on the information obtained
during the screening process, and in accordance with the Department's Regulations, the report has
been Screened Out for the following reason(s):
❑ The report did not involve a child or the allegations are not within the Department's
mandates
❑ The person allegedly responsible was not a caretaker of the child
f l The specific incident or condition is known to the Department and has been addressed by
the family
❑ The specific incident or condition is known to the Department and is currently being
addressed through the family's ongoing casework
❑ The reported allegations are due solely to a caretaker's inadequate economic resources or
to the existence of a handicapping condition
E. The only issue is maternal use of an appropriately prescribed medication resulting in a
substance exposed newborn; AND it has been verified that the mother is using the
medication as part of treatment as authorized by a qualified medical professional
