1, Lester Stein, being a Massachusetts Registered Architect Massachusetts (License #10446) hereby certify
that addition was constructed in accordance with the construction documents and building permits and
that it complies with applicable codes including those requirements set forth in
Massachusetts Building Code, 780 CMR.
1 have performed the necessary professional services for the entire project as required under 780 CMR
Section 107.6.2.1. 1 or my designee(s) shall be responsible for the following as specified in section
107.6.2.2:
1. Review, for conformance to the design concept, shop drawings, samples, and other submittals
which are submitted by the contractor or owner in accordance with the approved construction
documents.
2. Review and approve the control procedures for all code - required controlled materials.
