Store any collected lead -based paint waste from renovation activities under containment, in an enclosure, or behind a barrier that prevents dust and debris and prevents access to the waste, at the conclusion of each work day. B. Contain waste to prevent identifiable releases of dust and debris during transportation. C. Dispose of waste materials, whether hazardous, non- hazardous or regulated in accordance with all laws, and the provisions of this Section and any or all other applicable federal, state county or local regulations and guidelines. Ensure compliance with all laws and regulations relating to disposal. D. Containerize waste to prevent the release of dust and debris before removing the waste from the work area, and store the waste in a location protected from inadvertent disturbance. E. Non - Hazardous Materials 1. Contact the regional EPA, State and local authorities to determine disposal requirements f construction and demolition debris that contains lead paint (non - hazardous). Provide dumpsters /containers required for collection and disposal of such material as well as disposal in an approved landfill, F. Recyclable Materials (Non - Hazardous) 1. Demolition material deemed to be recyclable by the Contractor may contain lead, which could result in the recycling facility rejecting acceptance regardless of the TCLP result. The Contractor is hereby notified of this fact and shall bear all responsibilities and costs associated with acceptance and /or rejection of such materials in a C &D landfill, waste disposal facility and /or a recycling facility under their Base Bid. G. Hazardous Waste /Regulated Materials 1. Dispose of materials which are determined to be hazardous waste or regulated waste for lead as specified herein. Perform representative TCLP tests of demolition debris to ensure the material is properly profiled for disposal. This shall also include all testing required by the disposal or recycling facility. All costs associated with TCLP testing to profile the waste material shall be borne by the General Contractor. If the material is found to be hazardous waste or regulated waste, the General Contractor shall provide appropriate drums /containers for use. The General Contractor shall properly handle and transport all hazardous waste or regulated waste material into the drums /containers provided. The Owner shall coordinate all hazardous waste or regulated waste transfer and disposal procedures. END OF SECTION 13281 98 King Street, Northampton, MA Project No. 2901 Section 13281— Lead Paint - 6 LEAD PAINT c. Three stage decontamination unit must be established consisting of an Equipment Room, Shower, and Clean Room in series. 3.4 FIELD QUALITY CONTROL A. Site Tests, Inspection 1. Personal Air Sampling a. Perform negative exposure assessment on each trade subject to the OSHA Regulation. The assessment shall take place during routine work activities, which will simulate employees, actual exposure levels to lead. All assessments shall take place over an 8 -hour time period and shall include all appropriate PPE and biological monitoring required as stated herein and as required under 29 CFR 1926.62 Regulations. 1) Initial Determination: The General Contractor shall determine, through personal exposure monitoring on the job site or through relevant documentation from other similar jobs, whether workers will be exposed to airborne lead at or above the OSHA Action Level and Permissible Exposure Limit. If exposures at or above the action level are documented, appropriate health and safety procedures identified herein shall be followed. If levels below the action level are documented, the General Contractor shall exercise an appropriate level of care to ensure that exposures above the action level do not occur. 2) Personal Exposure Monitoring: The General Contractor shall perform personal exposure sampling to monitor personal exposure levels to airborne lead, Samples shall be taken for the duration of the work shift or for eight hours, whichever is greater. Personal samples need not be taken every day after the first day if working conditions remain unchanged, but must be taken every time there is a change in the removal operation, either in terms of the location or the type of work. Sampling will be used to determine eight hour Time Weighted Averages (TWA), The General Contractor is responsible for personal sampling as outlined in OSHA Standard 29 CFR 1926,62. b. Conduct additional monitoring whenever there is a chance of equipment, process, control, personnel or a new task is initiated that may result in additional employees being exposed to lead at or at or abo the ac le or may result in employees airea0v exposed at or the action level being exposed above the PEL. c. Establish air monitoring frequency in accordance with the requirements set forth in 29 CFR 1926.62. 2. Post Renovation Cleaning Verification a, Perform post remediation cleaning verification for interior renovation work areas in accordance with 40 CFR Part 745. Post remediation cleaning verification is required to be performed by the project's Certified Renovator, b. Reclean the entire work area at no additional cost to the Owner until the area passes post remediation cleaning verification. 3.5 CLEANING A. At the completion of work, clean interior and exterior work areas until no dust, debris or residue remains. 1. Collect all paint chips and debris, and seal in a heavy -duty bag, 2, Remove and dispose of protective sheeting as waste. . B. Interior Renovations 1. Clean all objects and surfaces in the work area within two feet of the work area in the following manner: a. Clean walls starting at the ceiling and working down to the floor by either vacuuming with a HEPA vacuum or wiping with a damp cloth. b. Thoroughly vacuum all remaining surfaces and objects in the work area, including furniture and fixtures, with a HEPA va.cuum. 98 King Street, Northampton, MA Project No. 2901 Section 13281 — Lead Paint - 5 LEAD PAINT 3.2 PREPARATION A. Protection 1. Extend full cooperation to Owner in all matters involving the use of Owner's facilities. At no time shall the Contractor cause or allow to be caused conditions, which may cause risk or hazard to the public or other contractors. 2. Maintain the work site in a neat and orderly manner at all times, so as not to interrupt or infringe upon the work of other trades. Upon completion of work in any given area, remove material and equipment associated with the work not necessary to complete other phases of the work in that area. B. Post signs clearly defining the work area and warning occupants and other persons not involved in renovation activities to remain outside the work area. C. Contain the work area so that no dust or debris leaves the work area while the renovation is being performed. Interior and exterior containments shall be provided in accordance with 40 CFR 745 3.3 SPECIAL TECHNIQUES A. Prohibit work practices listed below: 1. Open -flame burning or torching lead -based paint 2. Use of machines that remove lead -based paint through high speed operation such as sanding, grinding, power planing, needle gun, abrasive blasting, or sandblasting, unless such machines are used with HEPA exhaust control 3. Operating a heat gun on lead -based paint at temperatures of 1100 Fahrenheit or higher B. Personal Hygiene Practices 1. Enforce and follow good personal hygiene practices, where exposures to airborne lead above the OSHA PEL occurs or may be expected to occur. These practices shall be performed until personal exposure sampling indicates that exposures are below the PEL at which time the General Contractor has the option to continue or discontinue the use of personal hygiene f _iliti These practices shall i include Hnt '. _ not be iiiiiitcu to the following: a. No eating, drinking, smoking, or applying of cosmetics in work area. Provide a clean space, separated from the work area, for these activities. b. Workers must wash upon leaving the work area. Provide wash facilities consisting of, at least, running potable water, towels, and a HEPA vacuum. Upon leaving the work area, each worker will remove and dispose of work suit, wash and dry face and hands, and vacuum clothes. c. Disposable clothing, such as TYVEK suits, and other personal protective equipment (PPE) must be donned prior to entering work area. A clean room will be provided for workers to put on suits and other personal protective equipment and to store their street clothes. Disposable suits shall be used once, then properly discarded. d. Provide a lavatory facility located adjacent to the work area. The eating and drinking area, clean room, and the lavatory facility must be maintained in a clean and orderly fashion at all times. The General Contractor will provide portable lavatories when needed and disinfect them daily. 2. If air- monitoring data gathered by the General Contractor shows that employees' exposure to airborne lead exceeds 50 ug /m3, the following conditions apply: a. Showers must be provided. Shower water must pass through at least a 5.0 micron filter before returning to the public waste system. b. Workers must shower upon leaving work area. 66 King Street, Northampton, MA Project No. 2901 Section 13281— Lead Paint - 4 LEAD PAINT 1.6 QUALITY ASSURANCE A. Perform the disturbance of lead- containing paint using persons who are knowledgeable, qualified, and trained in the removal, treatment, handling, and disposal of such materials, and subsequent cleaning of the affected environment. B. Maintain the following records on site and available for review by all employees and authorized visitors until final completion: 1. Records on each employee on site including: a. Respirator fit -test documentation b. Training certificates c. Latest medical surveillance records d. Employee qualifications e. Copies of applicable permits and notifications. f. Personal air sampling results. g. MSDS sheets for all chemicals used during the project h. Copies of Contractor's written Hazard Communication and Respiratory Protection Programs, if required 1.7 PROJECT /SITE CONDITIONS A. Existing Conditions 1. The following documents are available to the Contractor: a. Asbestos Survey, 98 King Street, Northampton, MA - O'Reilly, Talbot & Okun Associates, Inc. (March 18, 2009). b. Phase I Environmental Assessment, Preliminary Asbestos /Lead -Based Paint Screening and Radon testing Report, 98 King Street, Northampton, MA - O'Reilly, Talbot & Okun Associates, Inc. (December 4, 2008). 2. Perform additional testing to confirm the presence of lead containing materials in the building in accordance with applicable regulations, Owner has performed a limited lead survey at the site but 100% testing of painted surfaces to confirm the presence of lead paint was not performed in the building, 1.8 SEQUENCING A. Coordinate the work of this section with that of other trades, in particular demolition work. 1.9 PHASING A. The work shall be conducted in the phases of the final schedule agreed upon by the Owner. PART 2 - PRODUCTS NOT USED PART 3 - EXECUTION 3.1 EXAMINATION A. Site Verification of Conditions 1. Verify and confirm quantity estimates, existing conditions and particulars of the work. 98 King Street, Northampton, MA Project No. 2901 Section 13281 — Lead Paint - 3 LEAD PANT 1. Written description detailing the means and methods for properly disposing of all demolition debris in accordance with local, state and federal regulations. 2. Written description detailing the means and methods to achieve compliance with the OSHA and EPA standard as well as the provisions outlined herein. 3. Qualifications a. Provide the following: 1) Documentation that all employees engaged in the work have received proper lead training, including a copy of current training certificates. Workers who will have the potential of lead exposure shall have proof of successfully completing a training course which covers the topics required by 29 CFR 1926.62 and 40 CFR Part 745 2) Documentation that firms performing renovations are certified in accordance with 40 CFR Part 745. 3) Name of individual assigned to fulfill the duties of Certified Renovator in accordance with 40 CFR Part 745. This individual shall also be responsible for ensuring compliance with the work practice standards of 29 CFR 1926.62 and be fully qualified in all aspects of safe work practices and procedures with lead containing materials. 4) Written Lead Compliance Program in accordance with OSHA 29 CFR 1926.62 5) Written Hazardous Communications Program including MSDS sheets for materials to be brought on -site. b. Provide the following if respiratory protection is to be worn during this project: 1) Written Respirator Program for review in accordance with 29 CFR 1920.103. 2) Records of successful respirator fit testing performed by a qualified individual within the previous 12 months, for each employee to be used on this project with the employee's name and social security number with each record. B. Closeout Submittals 1. Submit the following items to the Owner for approval prior to final payment: Original Copy of \'\'aste Disposal Manifests acknowli-dc-ling, disposal ` any hazardous and non-hazardous waste material from the project showing delivery date, quantity, and appropriate signature of landfill's authorized representative, 2. Copy of Personal Air Sample Results C. Substitution of materials or methods 1. Receive prior written approval from the Owner prior to any modifications to methods, procedures, and design. It is the intent of these documents to allow the Contractor to present alternative methods to the abatement processes herein, provided the following: a. Proposed modifications or substitutions to methods, procedures, or design comply with applicable regulations. b. Proposed modifications or substitutions are submitted for review in accordance with the requirements of the Project Manual and no later than 5 workdays prior to planned commencement of proposed modification. c. If the modification or substitution necessitates changes or additional work, assume the cost and the entire responsibility thereto unless performed under the approved Change Order Process. 2. Furnish the specifically named or designed items, methods or procedures designated in this Section unless requests for modification or substitution are made in accordance with the above instructions and any additional conditions provided in the Project Manual, supported by sufficient proof of — equality. a. The proposed use is a multi - family R -2 residence with more than four families and a hazard index of 4. There is a proposed inhabitable 258 square foot single story addition structurally attached to the southwest corner of the building. The porch and ramp systems will also be reconfigured. 3408.0 Structural The building needs to comply with the structural requirements of 3408 Requirements 3408.1 General The existing building is pre -1975. All additions and new structural work Requirements must comply with 780 CMR , 7t Edition. 3408.4 Level of Work The new structurally attached additions of 258 sq. feet comprise less than 10% of the existing 5580 sq. foot building. The brick foundation is unsuitable for subsurface construction and shows signs of collapse in the southeast corner, and therefor the entire foundation will be replaced. The wood framing has been partially rebuilt is in generally excellent shape, with little if any sag in the floors and no discernable racking. The roof structure appears to be excellent condition, but will be rebuilt to comply with the new needs of the building. The other major structural work to the existing building consists in providing a new system of columns and beams to replace the existing interior walls which are being relocated to accommodate the new design requirements. The rafters will need to be tied down to the wall framing by the use of hurricane clips. 3408.6.2.2.1 Geotechnical As noted above, the existing foundation is in good shape with no Review evidence of settlement. The soil is a medium dense sand and non - plastic silty sand with no danger of liquifaction and an allowable bearing capacity of 6000 lbs per sq. ft.
