Dear Patrick,
On February 1, 2012, the DPW received a transmittal of As -Built construction plans for Beaver Brook Estates.
The transmittal consisted of a cover letter, three plan plan sheets prepared by Heritage Surveys, Inc. dated
January 11, 2012, stamped and signed by Richard P. Weisse, P.E. and Bruce A. Coombs, P.L.S., and a CD with
unsealed plan sheets in AutoCad and PDF formats. The DPW considers these to be a preliminary submission
and will review them for accuracy and completeness when staff time allows. A request for revisions and /or
clarifications may be required before the plans are finalized.
DPW is aware that the existing 4 -unit condo has been connected to the new water and sewer utilities and that a
trench permit for these utility connections was issued in the fall of 2010 for a new house on Lot #8. Typically,
project closeout testing, certification and record information are required to be submitted before utility
connections are allowed. The approval of the connection permits for the exiting 4 -unit condo was necessary to
provide continuity, but the approval for Lot #8 was an oversight on the part of DPW.
Prior to the issuance of any new utility connection trench permits, the following testing, certification and
documentation are required for the completed project utilities.
• Sewer System - All tests to be witnessed by the Engineer of Record or his/her field representative and
results to be submitted to DPW for review and approval.
o Mandrel (Go- No -Go) test
o Video inspection with a copy of the DVD provided to the DPW
o Vacuum or exfiltration testing of all manholes
o Gravity system leakage testing (pressure or exfiltration). (If leakage testing is not feasible for
sewer pipe runs that are already connected and in service, then only video and mandrel testing
are required.)
o Stamped certification from a Massachusetts registered engineer that the sewer system was
constructed and tested in accordance with the approved plans.
