Vachula J.P.License #26988 28 Fifth Avenue Northampton, MA 01060 Senior Inspector Roth: I am in receipt of your letter, dated August 11, 2009, requesting information regarding a gas installation at 28 Fifth Avenue in Northampton, MA in November 2008. On October 30, 2008, I received an application to do a gas piping installation at the above - referenced address. This application was processed, and approved by me, as indicated by my initials on the front, left side of the permit. On November 31, 2008, I inspected the gas piping and witnessed a test of same with Scott Vachula present. The test and piping was satisfactory and was passed by me. At that time, I made Scott aware of C.O. detector requirements and questioned combustion air. Additionally, I was not contacted by the installing Plumber /Gas Fitter stating that the work was ready for a final gas inspection. As a result, I was not aware of any gas venting issues. Also, no permits were received by my office for any corrective work. However, the correspondence I received from the Adams Stove Company states that, on July 10, 2009, the owner of Adams Stove, along with a plumber removed the stove at the request of the home owner, Bonnie Cueman. Enclosed with this letter are: 1) A copy of the gas permit application, dated October 30, 2009, with my initials, allowing the gasfitter to begin work. Please review the reverse side of the permit for my notes and comments. 2) A copy of a letter from Adams Stove Co., dated July 24, 2009, and received in my office on July 27, 2009.
