The board has GRANTED your petition to access your machine room thru your women's restroom provided access is not thru a woman's toilet area. In accordance with the provisions of M.G.L. Chapter 143, Section 70(b), within thirty days of receipt any decision or order of the Board of Elevator Regulations, any person(s) aggrieved thereby may file an appeal to the Board of Elevator Appeals established under Chapter 22, Section 11A of Massachusetts General Laws. Any variance granted at our meeting is only for the specific section of the code referred to above and does not include any other section of 524 CMR the Massachusetts Elevator Code or any other location. J Elevator Pit As- Builts Christ United Methodist Church June 17, 2009 Construction joints without keyways or prepared surfaces are known as cold joints and are limited in their ability to transfer shear forces across the joint. Drilled and grouted dowels with shallow embedment of 12" or even less are not able to develop the full strength of the rebar in tension, and so can slip when it is under stress. The fact that the wall thickness was increased and the rebar was placed in a double -layer grid is good. According to my calculations, the pit walls will be able to support the lateral pressures from the wall loads. But the cold joints and short dowels could result in some slight flexing of those adjoining sections of wall. Furthermore, I cannot predict whether or not there would be some movement of the existing wall because of the lack of underpinning. The combination of the uneven bearing conditions for the existing footing, and potential voids created by concrete slump and shrinkage could cause some settlement. While it is doubtful that this settlement would result in a building collapse, there could be cracking of walls and /or slabs in these areas as the building is loaded with re- occupancy as well as with snow on the roof. HELLWIG, PE • STRUCTURAL ENGINEER • June 17, 2009 Jeffrey Scott Penn, Architect 77 Worthington Road Huntington, MA 01050 i spsed@verizon.net JU 1 - 2009 Re: Elevator Addition Christ United Methodist Church 271 Rocky Hill Road Northampton, MA Jeff, The following is a summary of my review of the changes to the pit construction, as shown in as- built sketches provided by Wright Builders on 6/2/09, 6/8/09 and 6/15/09, and additional information supplied via email on 6/15/09 and 6/16/09. As you know, my design drawings for the elevator installation showed certain key elements, including underpinning for the existing foundation walls adjacent to the pit, to carry the existing wall loads down to the elevator pit slab level. Also, the new steel - reinforced concrete walls were to be poured monolithically around the pit. As you also know, Wright Builders decided against the underpinning. They left the existing foundations in place, and cast two of the pit walls as one -sided pours up against the excavated earth under the existing footings. These walls extend partially under the existing footings. The walls are a minimum of 12" thick and have double -layer grids of #4 reinforcing bars spaced 16" on center. Also, the walls were poured in several stages. Construction joints between successive pours did not utilize keyways, roughened joint faces, or bonding agents. Dowels for horizontal reinforcing were drilled and grouted. Wright Builders did not provide adequate notification for review of these changes, and so this analysis is being made after the fact. The fact that the new pit walls were poured partially under the existing footings, means that there is uneven support underneath these footings. As well, concrete tends to slump as it is poured, and to shrink as it cures, meaning that there is a potential for void spaces under these existing walls. For this reason, the underpinning procedure typically leaves an intentional gap at the top which can be packed tightly with non - shrink grout. Because the existing walls were not underpinned, the vertical loads in the walls can exert lateral pressures on the side of the pit walls via the soil. These pressures are several times greater than the typical lateral pressures from normal backfill, so that typical rules of thumb for sizing foundation walls such as in basements do not apply. The following modifications were made to the design in these Drawings: 1. Underpinning of the existing foundations adjacent to the elevator pit was not performed per the design. The GC considered that the excavations were stable enough during the construction of the pit to avoid the underpinning work. The pit concrete wall was increased in thickness at that location, and the rebar was doubled. 2. The pit walls were poured in several sections, with both vertical and horizontal cold joints that had not been detailed on the Drawings. 3. The hoistway vent was relocated to the edge of the shaft, and the header along with it. As -built sketches of the changes are available from the G.C. Attached is the report of my review for items 1 and 2 above. Shop drawings were submitted and reviewed for steel beams and for the elevator cab. Comments were sent to the Architect. The work has progressed as planned with exemplary techniques and conscientious detailing. The elevator shaft is complete and ready for the elevator installation. A variance was sought and granted for the location of the mechanical room within the women's lavatory (a copy is included with this report). Baystate elevator thought that the location might not be compliant and recommended the variance. The elevator will be delivered and installation begun next week. The new walkways have been completed as have the area of refuge modifications, the office modifications, the sanctuary trimming and painting and the organ loft access door. The new basement egress hall and classroom are complete as is the lower level elevator access. The widened egress doors have been installed and the masonry repaired. The Air Conditioners are installed and one working, one nearly ready. The foyer planter has been removed and the guard constructed; the skylight shaft is ready for the glass installation. The brick spire assessment revealed degraded bell supports and the bells have been removed and stored in a garage on site for future display. Work has progressed as anticipated on the Church according to approved plans with the notable exception of the elevator pit area. This area was accomplished differently from the approved plans due to discovery of well compacted soil and a missing footing under the hall bearing wall. The contractor proceeded with the concrete work without prior approval of the Architect and Engineer, but with subsequent engineering assessment of the solution as outlined in Ryan Helwig's 17 June letter (Ryan will forward a copy of this letter with his report). The owner accepts the analysis and solution as safe. The sitework has also included rough preparation and compaction of the two ramp areas. The new A/C condenser pads are formed and the one at the lower level at the rear has been relocated to the east of the door due to the existing line locations. At the roofing near the tower, degraded sheathing material has been uncovered and replaced. Severe ice damming was noted this year at the eaves and near the skylight and remedial measures will include installation of proper venting and insulation at the eaves and new insulation at the skylight shaft walls. The septic design is still underway. The existing conditions will be revealed for the Engineer's review next week including soil conditions and existing footings. The draft schedule is to bring the project to completion approximately 17 September. One activity of note will be the vacating of the facility during August to facilitate sanctuary and other finish work; the regular services will be held at Laurel Park. It is my intention to furnish these reports at the end of the concrete work, at the end of the structural work, at the end of the mechanical work and at the request for certificate of occupancy (project completion).
