17D-012 (28) Anthony Patillo Subject: RETURN TO SCENE Start: Fri 9/16/2005 9:00 AM End: Fri 9/16/2005 9:30 AM Recurrence: (none) I returned to scene and met with FD, Board of Heath agent and Managment from Meadowbrook. Managment was informed that all apartments on lower level in Building 4 would not be allowed to return to apartments until they were cleaned. The cleanup would entail remoiving all carperts and removing sheetrock to a heaigh of at least two feet to allow inspection, if inspection showed higher levels that water had reached then more sheetrock would have to be removed. The apartments could not be re- occupied until Board of Health had inspected and cleared them for occupation.Managment agreed and would put up temporary barriers on staircases to prevent anyone from entering lower levels while clean up occured. They also agreed to clean the carpets in common areas where people had walked after being on lower levels. 1 Anthonatillo Subject: 491 Bridge road return to scene Start: Thu 9/15/2005 9:00 PM End: Fri 9/16/2005 1:00 AM Recurrence: (none) I monitored radio communication between public safety and FD. FD reported situation was worsening and more water was entering apartments. I returned to 491 Bridge road building 4. Upon arrival I went into building and entered lower apartments with FD. At this point there was very strong oders coming from the apartments. I asked that the apartments be evacuated and air quality be monitored for sewer gases, that the apartments should be declared un safe. FD immediately evacuated building. Red cross had been called. Mayor Higgings was called to inform her of situation as well as Concilor Alex Gheslin. The cities DPW was called to see if they could assit Meadowbrook managment. DpPW tried to ram jet line but was unsucssessful, DPW requested that line be drained. A call was put into Gregs septic service for their 5000 gallon truck.This was around midnight. The driver was in Colrain and had to go to Sunderland to get truck. I left scene at 1pm truck had not arrived, building 4 and 5 had been cleared. 1 Anthon itillo Subject: PUBLIC SAFETY CALLED AT HOME FLOODED APARTMENT BUILDING 4 491 BRIDGE ROAD REQUESTED TO RESPOND TO SCENE Start: Thu 9/15/2005 6:30 PM End: Thu 9/15/2005 8:00 PM Recurrence: (none) Responded to scene found FD in building 4 (lower left facing building) . FD had shut water main off to building (shut off located below stair) and had shut off water to toilet in bathroom. Water was still flowing from below toilet . I informed FD that this wa not a water problem but a sewer back up problem and that water flowing into bathroom was cming from outside of building from sewer line. We checked other apartments on lower level (4units) water had risen in bathtubs , water was discolored and had an order. I asked if maintence had been called , FD said they were responding from CT and would be on scene within 45 minutes. Whem Maintence arrived I told them they needed a plumber asap. Plumber arrived at approx 7:30 agreed it was a swer back up and went to cleanout located within building and then to street where he removed manhole cover and found manhole filled with sewer waste.Board of Heath was called because of public health hazard. Health agent arrived and gave plumber until midnight to resolve blocKage, plunbver felt he could clear blockage within hour. I left scene and returned home. . 1 7 V PIr/lk /VI 2-1=2.
