Box 169 Northampton, MA 01061 RE: Valley Motorsports Project Gentlemen: As of October 25, 1986 the following construction has been completed by the Edward J. O'Leary Company, inspected and found to be in compliance with applicable codes and construction documents for the project: 1. Site clearing and pavement removal has been completed in the area of the building addition. A rear entrance area has also been demolished by a separate contractor employed directly by the owner. 2. Foundation excavation and backfill for the building area has been completed. All footings rest on undisturbed soil with bearing capacities that meet construction document requirements. Gravel for the slab has been installed and compacted. 3. All concrete footings and walls have been completed. Two sets of endwall anchor bolts were found to be installed after the concrete wall was completed. These bolts however appear to be adequately installed. 4. OF 9UILOING INSPECTIONS Northampton, MA 01061 NORTHAMPTON, MA. 01060 RE: Valley Motorsports Construction Report #2 Gentlemen: Since my last report dated October 25, 1986, the following construction has been completed by the Edward J. O'Leary Company, inspected and found to be in compliance with applicable codes and construction documents for this project: 1. Site work and grading around the new building addition has been completed except for the paving around the rear of the new building. 2. All concrete work has been completed including footings, foundations, slabs and concrete aprons. 3. The Butler building complete with metal roof, walls and trim has been delivered to the job site and inspected for damage. All components appear to be free of damage. 4. Erection of the Butler structural steel frame has begun. All columns and beams have been erected and bolted in place. Approximately 80 percent of roof purlins have been erected and bolted in place. Sufficient cross bracing and wall girts installation has been completed to provide adequate stability for the building under construction. It was observed that the interior columns do not line up with the anchor bolts and concrete piers. The columns appear to be sitting only half way on the pier with only two of the four original anchor bolts lining up. Additional anchor bolts have been added. I could not verify that the pier below grade had been extended to properly support the column since the slab and diamonds have been poured. Additionally, several end wall columns did not line up with their anchor bolts and additional anchor bolts have been drilled and properly installed. It was, however, observed that several new bolt holes have been burned through the end wall beam. Additional washers and structural support is required in these locations. Box 169 Northampton, MA 01061 RE: Valley Motorsports Construction Report No. 3 Gentlemen: Since my last report dated November 15, 1986, the following construction has been completed by the Edward J. O'Leary Company: 1. All site work, grading and paving around the new building addition has been completed. 2. All concrete work has been completed. 3. Erection of the Butler steel building has been completed including all structural steel roof, walls and trim work. Some minor flashing work between the existing and new building remains to be completed to eliminate all potential water leakage at the roof. 4. All overhead doors, personnel doors and windows have been installed and completed. 5. As indicated in my previous report, three interior columns do not appear to be sitting in the center of the concrete piers below. The concrete slab has been poured around the piers and additional anchor bolts have been added. I could therefore not verify the exact size and location of the piers below grade which support the columns. I have, however, been assured by the O'Leary Company engineers that the column supports and anchor bolts provided are adequate to support the Butler building column loads. It was also observed that several column anchor bolts have not yet been fully tightened.
